VALOR DOS SERVIÇOS HOSPITALARES COM INTERNAÇÃO DE IDOSOS POR DOENÇAS DEGENERATIVAS DE DISCO EM REGIÕES BRASILEIRAS NOS ÚLTIMOS 10 ANOS
A doença degenerativa do disco é um processo patológico que afeta a coluna vertebral com diminuição do número de células e dos constituintes da matriz extracelular. Pela relação com outras estruturas osteomusculares, essa degeneração gera consequências clínicas, como instabilidade estrutural, hérnia de disco, lombalgia, estenose espinhal e radiculopatia. Sua etiologia é multifatorial, influenciada por fatores genéticos, como o polimorfismo do gene receptor da vitamina D (VDR) e de interleucina-1, ambientais, como tabagismo, e dinâmicos, como sobrepeso, obesidade, atividades pesadas, má postura e traumas. Outrossim, o envelhecimento é um componente fisiológico associado a esse processo. O diagnóstico da discopatia é realizado através da associação clínica e do exame físico com exames de imagem, e o tratamento inclui medidas não farmacológicas, farmacológicas e cirúrgicas. Diante da alta taxa de internações por discopatias no Brasil, nota-se um impacto econômico significativo no Sistema Único de Saúde (SUS), que deve ser avaliado, dado que tais serviços hospitalares podem ser evitados por medidas preventivas. Este estudo é retrospectivo observacional e analisa as admissões hospitalares de pessoas a partir de 60 anos de idade que ocorreram no SUS, em regiões brasileiras, de janeiro de 2008 a janeiro de 2018. Os dados foram obtidos a partir do Sistema de Informações Hospitalares (SIH/SUS) pelo valor em serviços hospitalares. Os resultados apontam que o valor total investido, nesse período, em serviços hospitalares foi de R$ 159.511.654,61, o qual foi destinado, majoritariamente, para a faixa etária de 60 a 69 anos. Ademais, a região com maior investimento é a Sudeste, e a com menor é a Norte. Conclui-se, então, que, no Brasil, milhões de reais são investidos no apoio às urgências enquanto várias medidas profiláticas poderiam evitar o aparecimento precoce ou a intensa evolução da Doença Degenerativa do Disco e diminuir o impacto econômico tão significativo no SUS.

Abstract:
Degenerative disc disease is a pathological process that affects the vertebral spine with a decrease in the number of cells and constituents of extracellular matrix. Due to the relation with other musculoskeletal structures, this degeneration leads to clinical consequences, such as structural instability, herniated disc, back pain, spinal stenosis and radiculopathy. Its etiology is multifactorial, influenced by genetic factors, as the polymorphism of the vitamin D receptor gene (VDR) and interleukin-1, by environmental factors, as smoking, and dynamic factors, such as overweight, obesity, heavy activities, bad posture and traumas. Furthermore, aging is a physiological component associated with this process. The diagnosis is through the association between clinical and physical examination with imaging exams, and the treatment includes non-pharmacological, pharmacological and surgical measures. In face of the high amount of hospitalization because of discopathies in Brazil, a significant economic impact is noticed in SUS (Sistema Único de Saúde), that must be evaluated, given that such hospital services can be avoided by preventive measures. This is a retrospective observational study and analysis of hospital admissions on SUS of people over 60 years old, in brazilian regions, from January 2008 to January 2018. The data was obtained from Sistema de Informações Hospitalares (SIH/SUS) for the value in hospital services. The results show that the total invested value, in this period of time, in hospital services was R$ 159.511.654,61, which was mainly intended for the age group of 60 to 69 years old. Also, the region with the highest investment is the Southeast, and the one with the lowest is the North. It is concluded that, in Brazil, millions of reais are invested in supporting emergencies while several prophylactic measures could prevent the early appearance or the intense evolution of Degenerative Disc Disease and reduce the significant economic impact in SUS.
