Abstract: The definition of hypertonic saline solution is one that has a concentration greater than 0.9% NaCl. Several clinical studies point to the benefits of using hypertonic saline solution for nasal washing in various sinonasal diseases. However, evidence from clinical studies involving the superiority of hypertonic saline solution over isotonic solution is controversial. Among the studies analyzed, the majority demonstrate that the use of hypertonic saline solution would bring benefits when compared to isotonic saline solution. Patients under 18 years of age showed greater beneficial effects from the use of hypertonic saline solution when compared to adults. Patients with rhinitis showed greater benefits than patients with rhinosinusitis. The benefits are greater in non-surgical patients than in post-operative patients. Furthermore, patients who use large-volume nasal irrigation and hypertonicity less than 5% experience greater benefits. Nasal washing with hypertonic saline solution showed greater benefits when compared to the use of isotonic saline solution in most sinonasal diseases, especially when used by patients under 18 years of age and in a concentration lower than 5%. Despite the studies presented, there is still no robust evidence for the dissemination of the use of hypertonic saline solution, requiring further studies on the subject.
