SISTEMATIZAÇÃO DA ASSISTÊNCIA DE ENFERMAGEM AO CLIENTE COM DOENÇA RENAL EM CUIDADOS PALIATIVOS
A Sistematização da Assistência de Enfermagem (SAE) e o exame físico são indispensáveis ao cliente com Doença Renal seja ela aguda ou crônica (conhecida como LRA ”lesão renal aguda” ou DRC “doença renal crônica”). A DRC vem aumentando de forma gradativa, sendo assim necessária a aplicação da SAE e a realização do exame físico corretamente. Trata-se de uma revisão bibliográfica
integrativa, onde buscamos publicações cientificas nas seguintes base de dados: LILACS, SciELO e BDENF, as publicações foram organizadas através da seleção das publicações, segundo estratégia PRISMA passando por 4 etapas, sendo elas: Identificação, Triagem, Elegibilidade e Inclusão. Foram identificados 1.047 registros
nas bases de dados entre BDENF, LILACS e SCIELO. Resultando após a seleção das publicações 15 artigos que responderam à pergunta de pesquisa. Os autores dos artigos totalizaram 70 pessoas, destas, 63 são enfermeiros, o que mostra que esta temática tem sido um ponto de reflexão e investigação para o mesmo. Dos 15 artigos
selecionados, 73,3% dos artigos abordaram D.E, e 60% abordaram intervenções de enfermagem, dentre os 15 artigos obtivemos um total de 55 D.E, observamos que os mais prevalentes foram: Volume de líquidos excessivos estando presente em 53,3% dos artigos, e apenas 20% dos artigos tinham propostas de intervenções de
enfermagem para este diagnóstico, entre os artigos selecionados ressaltamos também 3 artigos que apresentaram propostas de validação da SAE e 3 que apresentaram intervenções referente as complicações durante a hemodiálise. Assim concluímos que existem muitas literaturas referente à assistência de enfermagem ao DRC, porém, poucas trazem as fases da SAE e a relação entre elas, com isso notase a escassez de artigos que apresentam D.E, plano e prescrição, haja vista ser estas três fases da SAE, o que dificulta assim, a aplicação da mesma, visto que a maioria dos autores são enfermeiros e a SAE é privativa destes profissionais, ressalta-se a
necessidade e importância de aprimorar e aprofundar conhecimento e preparo para essa prática, visando assim, uma melhor assistência.

Abstract:
Systematization of Nursing Care (SNC) and physical examination are indispensable for clients with kidney disease, whether acute or chronic (known as AKI "acute kidney injury" or CKD "chronic kidney disease"). CKD has been gradually increasing, making it necessary to apply SNC and perform the physical examination correctly. It is an integrative bibliographic review, where we search for scientific publications in the following databases: LILACS, SciELO and BDENF, the publications were organized through the selection of publications, according to the PRISMA strategy, going through
4 stages, namely: Identification, Screening, Eligibility and Inclusion. 1,047 records were identified in the databases between BDENF, LILACS and SCIELO. Resulting after the selection of publications 15 articles that answered the research question. The authors of the articles totaled 70 people, of whom 63 are nurses, which shows that this theme has been a point of reflection and investigation for the same. Of the 15 selected articles, 73.3% of the articles addressed N.D, and 60% addressed nursing interventions, among the 15 articles we obtained a total of 55 N.D, we observed that the most prevalent were: Excessive fluid volume being present in 53.3 % of the articles, and only 20% of the articles had proposals for nursing interventions for this diagnosis, among the selected articles we also highlight 3 articles that presented validation proposals for SNC and 3 that presented interventions related to complications during
hemodialysis. Thus, we conclude that there are many literature referring to the nursing assistance to CKD, however, few bring the phases of SNC and the relationship between them, with that it is noted the scarcity of articles that present N.D, plan and prescription, considering these are three phases of SNC, which makes it difficult to apply, since most authors are nurses and SNC is private to these professionals, emphasizing the need and importance of improving and deepening knowledge and preparation for this practice, thus aiming , better assistance. -
