SÍNDROME DE RAPUNZEL: UMA CAUSA RARA DE DOR ABDOMINAL
A Síndrome de Rapunzel representa uma entidade rara que acontece quando os tricobezoares, representado pelo acúmulo de cabelos no tubo digestivo, ocupam o estômago e porções inicias do intestino delgado. Os sintomas são muito variáveis e dependem de sua localização e tamanho. Geralmente está associada a tricotilomania, que é compulsão por arrancar o cabelo, e a tricofagia, que é a ingestão de cabelo. Este relato descreve um caso de um paciente de 18 anos, do sexo feminino, que procurou a emergência com queixa de náuseas e vômitos há uma semana, com evolução para dor abdominal difusa nas últimas horas. Ao exame físico, apresentava-se em bom estado geral, com massa palpável, endurecida e móvel em região epigástrica, sem sinais de irritação peritoneal. A Tomografia Computadorizada de Abdome revelou sinais de discreto pneumoperitônio e acentuada gastrectasia determinada por exuberante massa intraluminal de conteúdo heterogêneo, amonteado, apresentando extensão além do piloro, em contiguidade para o intestino delgado, ratificando a suspeita de Síndrome de Rapunzel, sendo, então, submetida à gastrotomia transversa para retirada de massa que ocupava toda a câmara gástrica e ainda 1º e 2º porções duodenais.

Abstract:
Rapunzel Syndrome is a rare entity that happens when the trichobezoars, represented by the accumulation of hairs in the digestive tract, occupy the stomach and early portions of the small intestine. The symptoms are very variable and depend on their location and size. It is usually associated with trichotillomania, which is compulsion to tear the hair, and trichophagia, which is the ingestion of hair. This report describes a case of an 18-year-old female patient who sought the emergency complaining of nausea and vomiting a week ago, with progression to diffuse abdominal pain in the last hours. At physical examination, the patient was in good general condition, with a palpable mass, hard and mobile in the epigastric region, with no signs of peritoneal irritation. Abdomen Computed Tomography showed signs of discreet pneumoperitoneum and marked gastrectasis determined by exuberant intraluminal mass of heterogeneous content, which was enlarged beyond the pylorus, in contiguity to the small intestine, ratifying the suspicion of Rapunzel Syndrome, and was then submitted to the transverse gastrotomy for removal of mass that occupied the entire gastric chamber and also 1 st and 2 nd duodenal portions.
