RESSECÇÃO DE GLIOMA INSULAR: A CIRURGIA E O PÓS-OPERATÓRIO – UMA REVISÃO BIBLIOGRÁFICA
O objetivo dessa pesquisa é a discussão sobre a formação de gliomas no lobo da ínsula e apresentar as dificuldades da ressecção cirúrgica nesse local enfrentadas pelos neurocirurgiões, bem como as chances de sobrevida dos pacientes. A revisão bibliográfica ocorreu partir de leitura de artigos científicos encontrados na base de dados PubMed e no Jornal Brasileiro de Neurocirurgia e livros acadêmicos impressos da especialidade médica de Neuroanatomia no período de 2008 a 2020. A pesquisa foi restringida pelas palavras-chaves: “Glioma”, “Lobo da ínsula”, “Ressecção cirúrgica”. Foram consultados 1 livro acadêmico e 171 artigos científicos por meio de buscas manuais. Utilizando como critério de restrição o intervalo de tempo 2008-2020, foram selecionados 138 artigos no PubMed, 1 no Jornal Brasileiro de Neurocirurgia e 1 no Arquivo Brasileiro de Neurologia. Restringindo o método de estudo como Revisão Sistemática, foram lidos 9 artigos. Por conseguinte, foram selecionados e lidos 1 livro acadêmico e 6 artigos científicos tendo como critério de inclusão aqueles que continham as palavras-chaves e os filtros de restrição. Ao final, com base na pesquisa realizada, tem-se como principais tópicos abordados na conclusão da pesquisa os gliomas insulares de malignidade de baixo grau sendo lesões complexas nos campos técnico-cirúrgico e terapêutico, uma vez que muitas delas são assintomáticas, e que embora a ressecção cirúrgica continue a ser um desafio para o neurocirurgião, é necessária para o paciente com glioma insular de malignidade de alto grau.

Abstract:
The objective of this research is to discuss the formation of gliomas in the insular lobe and to present the difficulties of surgical resection in that location faced by neurosurgeons, as well as the patients' chances of survival. The bibliographic review was based on the reading of scientific articles found in the PubMed database and in the Brazilian Journal of Neurosurgery and printed academic books of the medical specialty of Neuroanatomy in the period from 2008 to 2020. The search was restricted by the keywords: “Glioma” , “Insular lobe”, “Surgical resection”. One academic book and 171 scientific articles were consulted through manual searches. Using the 2008-2020 time interval as a restriction criterion, 138 articles were selected in PubMed, 1 in the Brazilian Journal of Neurosurgery and 1 in the Brazilian Neurology Archive. Restricting the study method to Systematic Review, 9 articles were read. Therefore, 1 academic book and 6 scientific articles were selected and read with the inclusion criteria those that contained the keywords and the restriction filters. In the end, based on the research conducted, the main topics addressed at the conclusion of the research are low-grade malignant island gliomas, which are complex lesions in the technical-surgical and therapeutic fields, since many of them are asymptomatic, and although surgical resection remains a challenge for the neurosurgeon, it is necessary for the patient with high-grade malignant insular glioma.
