RELATO DE CASO: PNEUMOTÓRAX CATAMENIAL
Introdução: o pneumotórax catamenial é o acúmulo de ar na cavidade torácica durante o período menstrual, o qual acomete exclusivamente o sexo feminino, no período da menacme. Trata-se de uma condição rara, que geralmente ocorre no hemitórax direito, cursando com dor torácica do tipo pleurítica, podendo ou não estar associada a dispneia e tosse, manifestando-se também como hemotórax ou hidropneumotórax. O diagnóstico pode ser feito clinicamente com auxílio de exames de imagens e também com avaliação anatomopatológica. Algumas vezes o diagnóstico patológico não é obtido, o que não impede o tratamento, que consiste em terapia hormonal ou até mesmo abordagem cirúrgica visando sempre a cessação dos episódios de pneumotórax e das dores, responsável por uma péssima qualidade de vida. O objetivo deste trabalho é relatar um caso desta enfermidade rara, a fim de propiciar enriquecimento literário. Descrição: a paciente, de 35 anos, apresentou o primeiro episódio de pneumotórax durante a gestação, necessitando drenagem de tórax. Houve recorrência de novos eventos de pneumotórax, partindo-se, portanto, para a investigação, a qual resultou na hipótese diagnóstica de pneumotórax catamenial. A paciente foi então submetida a terapêutica conservadora e também cirúrgica. Discussão: a endometriose pélvica é um dos achados esperados no pneumotórax catamenial, entretanto, no relato descrito não foi feito o diagnóstico anátomo-patológico. Outros fatores encontrados, como a clínica e a imagem permitiram o diagnóstico da comorbidade. Conclusão: frente a doenças incomuns é essencial ter conhecimento de outros relatos de caso para direcionar o diagnóstico e avaliar a melhor conduta terapêutica, sendo isso feito por análise de perspectivas diferentes da comorbidade em questão.

Abstract:
Introduction: catamenial pneumothorax is the accumulation of air in the thoracic cavity during the menstrual period, which affects only the female sex, in the period of menacme. It is a rare condition, which usually occurs in the right hemithorax, coursing with pleuritic chest pain, which may or may not be associated with dyspnea and cough, also manifesting as hemothorax or hydropneumothorax. The diagnosis can be made clinically with the aid of imaging exams and also with anatomopathological evaluation. Sometimes the pathological diagnosis is not obtained, which does not prevent treatment, which consists of hormonal therapy or even a surgical approach, always aiming at the cessation of pneumothorax episodes and pain, responsible for a poor quality of life. The objective of this work is to report a case of this rare disease, in order to provide literary enrichment. Case description: the 35-year-old patient had the first episode of pneumothorax during pregnancy, requiring chest drainage. There was a recurrence of new pneumothorax events, therefore, investigation was carried out, which resulted in the diagnostic hypothesis of catamenial pneumothorax. The patient was then submitted to conservative and surgical therapy. Discussion: pelvic endometriosis is one of the expected findings in catamenial pneumothorax, however, in the report described, the anatomopathological diagnosis was not made. Other factors found, such as clinic and image allowed the diagnosis of comorbidity. Conclusion: in the face of unusual diseases, it is essential to be aware of other case reports to guide the diagnosis and assess the best therapeutic approach, and this is done by analyzing different perspectives of the comorbidity in question.
