Abstract: Introduction: Elderly people are more predisposed to some diseases and problems, such as dysbiosis. The appropriate use of probiotic supplementation in elderly people is an effective strategy to improve intestinal health, involving the regulation of the intestinal barrier function, the activation of the immune system and the modulation of colon metabolism. The role of nurses in the administration of probiotics in institutionalized elderly people in their routine is essential, however, it requires more research, studies and reflections on the benefits and challenges in implementing probiotic supplementation. Objective: To identify, through a bibliographic review, the role of nurses and the challenges in the use of probiotics in institutionalized elderly people. Methodology: This was a descriptive bibliographic review research, constructed from materials published between 2018 and 2025. To select the texts, an online search was carried out in the BVS (Virtual Health Library) Database. Thirteen publications that addressed the study theme, published in full, with full texts available, in Portuguese were considered. The keywords investigated were: Probiotics; Elderly; Nursing. Results: Nurses play a fundamental role in the administration and monitoring of probiotic use in institutionalized elderly individuals. The effectiveness of the therapy depends on the choice of appropriate strains, doses administered and customization to meet the specific conditions of each patient. The implementation of protocols that include probiotics in ILPs, the lack of adequate training, resources and interdisciplinary collaboration is a challenge for the adoption of this therapy. Final considerations: The role of nurses in the use of probiotics in institutionalized elderly individuals is still a challenge, even in view of the benefits found, due to the lack of adequate training and interdisciplinary cooperation. More studies based on scientific research are needed, with the aim of presenting the benefits of probiotic therapy on the health of the intestinal microbiota in elderly individuals.
