Neutropenia febril, urgência onco-hematológica: revisão de literatura
Sendo o câncer uma doença multifatorial e com alta taxa de mortalidade a nível mundial, a prevenção, o diagnóstico precoce e o tratamento são fundamentais para evitar a mortalidade e aumentar as chances de cura da doença. Para isso é necessário, além do conhecimento sobre a doença, uma maior compreensão das terapias antineoplásicas utilizadas e principalmente das complicações oncológicas deste tratamento. O objetivo deste estudo foi conhecer, através de Revisão Bibliográfica, os achados da literatura sobre a neutropenia febril, que é considerada uma emergência oncológica, em pacientes onco-hematológicos. Realizou-se busca de estudos na base de dados PUBMED. Foram utilizados os descritores padronizados do DECS: Febrile Neutropenia and Hematology and Oncology. A partir desta busca, os estudos recuperados foram avaliados de acordo com a relevância ao tema proposto. Os estudos demonstraram que, mesmo sem um consenso com relação à definição de febre, o tratamento precoce é fundamental para um desfecho positivo. Um diagnóstico clínico-laboratorial rápido e preciso juntamente com o início da terapia medicamentosa adequada determinarão o sucesso do tratamento. Otimização do processo de trabalho e estrutura hospitalar e/ou ambulatorial organizada, envolvendo todas as partes da equipe assistencial, promovem um atendimento aos pacientes mais efetivo, seguro e eficaz.

Abstract:
As cancer is a multifactorial disease with a high mortality rate worldwide, prevention, early diagnosis and treatment are essential to avoid mortality and increase the chances of curing the disease. For this, it is necessary, in addition to knowledge about the disease, a greater understanding of the antineoplastic therapies used and especially the oncological complications of this treatment. The objective of this study was to know, through a Bibliographic Review, the findings of the literature on febrile neutropenia, which is considered an oncological emergency, in onco-hematological patients. A search for studies was carried out in the PUBMED database. The DECS standardized descriptors were used: Febrile Neutropenia and Hematology and Oncology. From this search, the studies retrieved were evaluated according to their relevance to the proposed theme. Studies have shown that, even without a consensus regarding the definition of fever, early treatment is essential for a positive outcome. A rapid and accurate clinical-laboratory diagnosis together with the initiation of adequate drug therapy will determine the success of the treatment. Optimization of the work process and organized hospital and/or outpatient structure, involving all parts of the care team, promote more effective, safe and efficient patient care.
