METEMOGLOBINEMIA POR USO DE DAPSONA: UM RELATO DE CASO
A metemoglobinemia é uma desordem caracterizada pela oxidação da hemoglobina a qual compromete o transporte de oxigênio corpóreo. A patologia pode ter origem congênita ou adquirida, sendo essa última decorrente do uso de alguns medicamentos, como a dapsona. O paciente apresenta-se com sintomatologia referente a queda da concentração de oxigênio nos tecidos, que pode ir desde de dispneia até o óbito. Seu diagnóstico ocorre mediante a mensuração dos níveis de metemoglobina, os quais devem ser menores que 5%. O caso em questão refere-se a uma paciente que apresentava baixa saturação de oxigênio, em contraste com o exame físico, aparentemente normal, denotando um diagnóstico precoce o que levou a um ótimo prognóstico. Assim, é necessário o reconhecimento da existência da metemoglobinemia, bem como de suas causas, a fim de que a suspeição clínica seja precocemente efetivada e o tratamento seja instaurado.

Abstract:
Methemoglobinemia is a disorder characterized by the oxidation of hemoglobin which compromises the transport of body oxygen. The pathology may have a congenital or acquired origin, the latter resulting from the use of some medications, such as dapsone. The patient presents with symptoms related to a drop in oxygen concentration in the tissues, which can range from dyspnea to death. The diagnosis is made by measuring methemoglobin levels, which should be less than 5%. The case in question refers to a patient who had low oxygen saturation, in contrast to the physical examination, apparently normal, showing an early diagnosis, which led to an excellent prognosis. Thus, it is necessary to recognize the existence of methemoglobinemia, as well as its causes, so that clinical suspicion arises early on, and treatment is initiated.
