MEDIDAS DE PREVENÇÃO DE PNEUMONIAS ASSOCIADAS À VENTILAÇÃO MECÂNICA EM UMA UTI PÚBLICA DE BRASÍLIA
Este trabalho investigou as pneumonias associadas à ventilação mecânica (PAV) e as medidas de prevenção (bundle) em uma UTI pública de Brasília. Trata-se de um estudo transversal, retrospectivo e prospectivo. A coleta de dados compreendeu os anos de 2017 e 2018 e foi realizada através da investigação de prontuários dos pacientes e fichas de acompanhamento diário da equipe da Comissão de Controle de Infecção Hospitalar (CCIH). Foram incluídos pacientes da UTI que permaneceram internados por um período superior a 24 horas, e com idade ≥18 anos, de ambos os sexos e em uso de aparelho de ventilação mecânica (VM). As variáveis investigadas foram sexo, idade, período de internação, desfecho clínico, pneumonias associadas à ventilação mecânica e pacote de medidas (bundle) para prevenção de pneumonias associadas à ventilação mecânica. O total de pacientes internados na UTI investigada em 2017 e 2018 foram 247, sendo que a amostra total dos pacientes que fizeram uso de ventilação mecânica neste período representou 83% (n=205). O número de pacientes em uso VM e sem PAV foi de 161 (78,53%) e os pacientes com VM e com PAV foi de 44 (21,46%). Houve uma prevalência do sexo masculino na amostra, desfecho de óbito elevado e um período de internação significativamente prolongado no grupo com PAV. Entre as medidas de prevenção de PAV investigadas, a elevação da cabeceira do leito à 30-45°, apresentou uma taxa de inadequação maior entre pacientes com PAV de forma estatisticamente significante. Esse trabalho aponta o perfil dos pacientes acometidos por pneumonias associadas à ventilação mecânica e a significante inadequação da elevação da cabeceira do leito, dentre as medidas de prevenção de PAV. Estes resultados, ensejam colaborar com a comunidade científica e equipes envolvidas na prevenção desses agravos, a fim de contribuir com enfrentamento desse importante problema de saúde pública.

Abstract:
This study investigated pneumonia associated with mechanical ventilation (VAP) and prevention measures (bundle) in a public ICU in Brasília. This is a cross-sectional, retrospective and prospective study. Data collection comprised the years 2017 and 2018 and was carried out through the investigation of patient records and daily follow-up sheets by the team of the Hospital Infection Control Commission (CCIH). Patients admitted to the ICU who remained hospitalized for more than 24 hours and aged ≥18 years, of both sexes and using mechanical ventilation (MV), were included. The variables investigated were sex, age, length of stay, clinical outcome, pneumonia associated with mechanical ventilation and a package of measures (bundle) to prevent pneumonia associated with mechanical ventilation. The total number of patients admitted to the ICU investigated in 2017 and 2018 was 247, with the total sample of patients using mechanical ventilation in this period representing 83% (n = 205). The number of patients using MV and without VAP was 161 (78.53%) and patients with MV and VAP were 44 (21.46%). There was a male prevalence in the sample, a high death outcome and a significantly prolonged hospital stay in the VAP group. Among the investigated VAP prevention measures, the elevation of the head of the bed to 30-45 °, showed a statistically significant higher rate of inadequacy among patients with VAP. This work points out the profile of patients affected by pneumonia associated with mechanical ventilation and the significant inadequacy of the elevation of the head of the bed, among the measures to prevent VAP. These results, aim to collaborate with the scientific community and teams involved in the prevention of these diseases, in order to contribute to facing this important public health problem.
