MANEJO DO PACIENTE ONCOLÓGICO RELACIONADO AO TRATAMENTO ANTINEOPLÁSICO FRENTE À COVID-19: REVISÃO INTEGRATIVA
Introdução: A COVID-19 registrou mais de um milhão de mortes no mundo, tendo os pacientes oncológicos como um grupo de risco, pois possuem tratamentos supressores do sistema imunológico e maior exposição pela necessidade de deslocamento para seguimento da terapêutica. Questiona-se a manutenção do tratamento desses pacientes, desafiando a reorganização de condutas sem comprometer seu prognóstico. Objetivos: Realizar um levantamento das pesquisas sobre a escolha pela continuidade ou não do tratamento oncológico associado ao manejo da COVID-19. Metodologia: Trata-se de um estudo de levantamento bibliográfico, descritivo, do tipo revisão integrativa da literatura, o qual utilizou-se os descritores “Infecções por Coronavirus”, “Terapêutica” e “Neoplasias” na base de dados da Biblioteca Virtual de Saúde (BVS). Incluiu-se estudos publicados em 2020 em inglês ou português que abordaram a temática estudada. Resultados e discussão: Foram selecionados 06 artigos para análise que tratam sobre as recomendações e as consequências do tratamento oncológico no contexto da pandemia. Dentre as recomendações, as medidas de higiene adequadas, a avaliação individualizada de cada paciente sobre o melhor tratamento a ser escolhido referentes QT, RT ou tratamento cirúrgico e a triagem contínua de sintomas típicos de covid-19 visando o seu reconhecimento precoce foram abordadas. Além disso, há evidências de que a pandemia trará consequências que irão impactar no prognóstico e no aumento da morbimortalidade de grande quantidade pacientes visto que o adiamento do cuidado ao câncer está associado a progressão da doença. Conclusão: A interrupção do tratamento do câncer leva a inúmeros prejuízos materiais e imateriais no cenário oncológico, devendo ser realizada a decisão do adiamento terapêutico de forma multidisciplinar, avaliando a relação risco/benefício da continuidade do tratamento.

Abstract:
Introduction: COVID-19 recorded more than one million deaths worldwide, with cancer patients as a risk group, as they have immune suppressive treatments and greater exposure due to the need to travel to follow up on therapy. The maintenance of the treatment of these patients is questioned, challenging the reorganization of conducts without compromising their prognosis. Objectives: Conduct a survey of research on the choice of whether or not to continue with cancer treatment associated with the management of COVID-19. Methodology: This is a study of bibliographic, descriptive, integrative literature review type, which used the descriptors "Coronavirus Infections", "Therapeutics" and "Neoplasms" in the database of the Virtual Health Library (VHL). Studies published in 2020 in English or Portuguese that addressed the subject studied were included. Results and discussion: 06 articles were selected for analysis that deal with the recommendations and consequences of cancer treatment in the context of the pandemic. Among the recommendations, the appropriate hygiene measures, the individualized assessment of each patient on the best treatment to be chosen regarding QT, RT or surgical treatment and the continuous screening of typical covid-19 symptoms aiming at their early recognition were approached. In addition, there is evidence that the pandemic will impact the prognosis and increase the morbidity and mortality of large numbers of patients since the postponement of cancer care is associated with disease progression. Conclusion: The interruption of cancer treatment leads to countless material and immaterial losses in the oncological scenario, and the decision to postpone the treatment in a multidisciplinary way must be made, evaluating the risk / benefit ratio of the continuity of treatment.
