MANEJO AO CLIENTE QUE SOFREU ESCALPELAMENTO: IMPLICAÇÕES AO CUIDADO SISTEMATIZADO DE ENFERMAGEM
A região amazônica possui uma imensa quantidade comunidades ribeirinhas, o que gera também inúmeros casos de escalpelamento devido a acidentes com embarcações. Objetivamos descrever a Sistematização da Assistência de Enfermagem (SAE) aplicada à paciente pediátrico, vítima de escalpelamento. Estudo descritivo do tipo relato de caso, com abordagem qualitativa, desenvolvido em um Hospital Público em Belém-Pará, com coleta realizada entre 28 de maio e 07 de junho de 2012 através da análise do prontuário, entrevista e exame físico. Os diagnósticos, prescrições e resultados de enfermagem foram estabelecidas de acordo com a literatura (NANDA, NIC e NOC). Os diagnósticos encontrados foram risco de infecção, dentição, conhecimentos deficientes a respeito da saúde e higiene bucal, risco de baixa autoestima, integridade tissular prejudicada, déficit no autocuidado, recuperação cirúrgica retardada relacionada à infecção. As intervenções consistem na lavagem das mãos antes e depois do atendimento e usar luvas; tornar o cliente autor de seu autocuidado; encaminhar para o dentista da instituição; encoraja-lo a expressar os sentimentos; promover a interação social; remover o tecido necrosado; limpar o leito da ferida; manter o horário e a rotina do banho; providenciar materiais para o banho; instruir o indivíduo e a família quanto às causas e riscos de infecção; manter ingesta calórica e proteica. Com isso espera-se que a cliente seja capaz de gerir o processo de melhora do seu quadro de saúde, reduzir o medo de mudanças permanentes na aparência e ajudar a preservar um autoconceito positivo, integridade da pele livre de infecção, além de melhora progressiva em seu quadro geral, se possível. A SAE mostra-se como ferramenta eficaz e auxilia no atendimento integral a criança vítima de escalpelamento, na qual engloba a pessoa com uma visão holística, traçando cuidados que visam desde o trauma físico, como também o trauma psicológico que a vítima venha a sofrer.

Abstract:
The Amazon region has an immense number of riverside communities, which also generates countless cases of scalping due to accidents with boats. We aim to describe the Nursing Care Systematization (SAE) applied to pediatric patients, victims of scalping. Descriptive study of the case report type, with a qualitative approach, developed in a Public Hospital in Belém-Pará, with collection carried out between May 28 and June 7, 2012 through the analysis of the medical record, interview and physical examination. Nursing diagnoses, prescriptions and results were established according to the literature (NANDA, NIC and NOC). The diagnoses found were risk of infection, teething, deficient knowledge about health and oral hygiene, risk of low self-esteem, impaired tissue integrity, deficit in self-care, delayed surgical recovery related to infection. Interventions consist of washing hands before and after care and wearing gloves; make the client author of their self-care; forward to the institution's dentist; encourages you to express your feelings; promote social interaction; removing necrotic tissue; clean the wound bed; maintain the bath schedule and routine; providing bathing materials; instruct the individual and the family on the causes and risks of infection; maintain caloric and protein intake. With this, it is expected that the client will be able to manage the process of improving her health condition, reduce the fear of permanent changes in appearance and help to preserve a positive self-concept, skin integrity free of infection, in addition to progressive improvement in your overall picture, if possible. SAE proves to be an effective tool and assists in the integral care of the child victim of scalping, in which it encompasses the person with a holistic view, tracing care aimed at the physical trauma, as well as the psychological trauma that the victim may suffer.
