IMUNOTERAPIA APLICADA À NEOPLASIA DE MERKEL METASTÁTICA EM PACIENTE IMUNOSSUPRIMIDO
Paciente do sexo masculino, 59 anos, com antecedente de transplante renal por amiloidose em outubro/2015 em uso de imunossupressores. Evoluiu com nódulo de crescimento progressivo em região malar direita, sendo submetido à ressecção com anatomopatológico compatível com carcinoma de células de Merkel - imunohistoquimica positiva para cromogranina e sinaptofisina. Após 5 meses, evoluiu com recidiva local tratada com ressecção cirúrgica. Apresentou nova recidiva após 2 meses, sendo submetido a ressecção local associado a esvaziamento cervical a direita com anatomopatológico relevando carcinoma de células de merkel com infiltração em glândulas parótidas e submandibular, margens comprometidas e metástases para 48/54 linfonodos avaliados. Iniciado radioterapia adjuvante, porém com progressão tumoral local durante o tratamento. Na ocasião, a doença foi considerada irressecável sendo iniciado avelumabe com rápida diminuição tumoral, boa tolerância e evoluindo com resposta completa após 3 ciclos de imunoterapia.

Abstract:
Male patient, 59 years old, with a history of kidney transplantation for amyloidosis in October/2015, using immunosuppressants. The patient evolved with a progressively growing nodule in the right malar region, and underwent resection with a pathological examination compatible with Merkel cell carcinoma - positive immunohistochemistry for chromogranin and synaptophysin. After 5 months, he evolved with local recurrence, treated with surgical resection. He presented a new recurrence after 2 months, and was submitted to local resection associated with neck dissection on the right with pathological examination revealing merkel cell carcinoma with infiltration in parotid and submandibular glands, compromised margins and metastases for 48/54 lymph nodes evaluated. Adjuvant radiotherapy was started, but with local tumor progression during treatment. At the time, the disease was considered unresectable and avelumab was started with rapid tumor shrinkage, good tolerance and progressing to a complete response after 3 cycles of immunotherapy.
