IMPORTÂNCIA DA INTEGRAÇÃO DE CONCEITOS TEÓRICOS PARA TRATAMENTO ADEQUADO: RELATO DE CASO
O líquen plano é uma doença dermatológica crônica que afeta a mucosa bucal. Essa patologia está diretamente ligada ao sistema imunológico do indivíduo, bem como seu nível de estresse e possíveis doenças que podem ser analisadas e descritas durante a anamnese. Basicamente, existem duas formas de lesões bucais: a reticular e a erosiva. A forma reticular é muito comum e assintomática, é caracterizada pela presença de estrias de Wickham e, não causa tantos danos ao paciente. A forma erosiva, caso do paciente em questão, apesar de não ser tão comum, é mais significante pois as lesões frequentemente são sintomáticas. Essa forma da doença pode manifestar-se associada a um tipo de gengivite, a descamativa. O corte histológico da lesão apresenta, sobretudo, um considerável infiltrado de linfócitos T. Por ser uma doença de causa idiopática, é necessário analisar o histórico do paciente, descrito em anamnese e exames complementares, para obter um diagnóstico preciso. O líquen plano erosivo é mais difícil de ser diagnosticado, pois não é totalmente visível em suas características clínicas. A biópsia é necessária para descartar outras doenças erosivas ou ulcerativas, como o lúpus eritematoso ou a estomatite ulcerativa crônica. Por se tratar de uma patologia imunomediada, os corticoides são a melhor opção para o tratamento. O objetivo deste trabalho é enfatizar a importância da investigação do histórico do paciente na anamnese, aumentando assim as habilidades dos profissionais de odontologia para melhores escolhas terapêuticas. Conclui-se, portanto, que o caráter investigativo e criativo através de conhecimentos prévios e levantamento de hipóteses embasadas na literatura, além do auxílio de outras áreas da saúde, sempre levarão ao sucesso do diagnóstico e tratamento adequado dos pacientes.

Abstract:
Lichen planus is a chronic dermatological disease that affects the oral mucosa. This pathology is directly linked to the immune system, incluiding stress levels and resident diseases obtained by analysis during the anamnesis. Basically, there are two forms of oral lesions: reticular and erosive. The reticular form is very common and asymptomatic, is characterized by the presence of Wickham striae and does not cause important damage to the patient. Although the erosive form is not so common, it is more significant because the lesions are often symptomatic. This disease form of the may be associated to some types of gingivitis. The histological section analysis of the lesion presented, above all, a considerable infiltrate of T lymphocytes. Being an idiopathic disease, it is necessary to analyze the patient's health history, described in anamnesis and complementary exams, achieving an accurate diagnosis. Erosive lichen planus is more difficult to diagnose because it is not fully visible in its clinical features. A biopsy is required to rule out other erosive or ulcerative diseases such as lupus erythematosus or chronic ulcerative stomatitis. Being an immunomediated condition, corticosteroids are the most important treatment option. The purpose of this study is to emphasize the importance of investigating the patient's history in the anamnesis, this way enhancing dental professionals skills to a better therapeutic choices. It is concluded, therefore, the investigative and creative character through previous knowledge and hypothesis based on the literature, and the link with other health areas, will always lead to the successful diagnosis and proper treatment of patients.
