IMPLANTAÇÃO DO TESTE DA LINGUINHA EM UMA MATERNIDADE DO RECIFE: RELATO DE EXPERIÊNCIA
Introdução: O Protocolo de Avaliação do Frênulo da língua em bebês possibilita padronizar os achados anatômicos do frênulo e seu impacto nas funções de sucção e deglutição durante a amamentação. Serviu de base para a Lei Nº 13.002 de 20 de Junho de 2014, publicada em Diário Oficial, tornando obrigatório sua realização nas maternidades do país, como parte integrante da Triagem Neonatal. Desta forma, as fonoaudiólogas do hospital foram capacitadas e implantaram, em 2015, o teste da linguinha, tornando-o pioneiro e referência na prestação desse serviço. Objetivo: Descrever a implantação do Teste da Linguinha em maternidade do Recife. Método: Relato de experiência descritivo sobre a implantação do teste da linguinha em maternidade particular. A equipe de Fonoaudiologia estruturou projeto para implantação, apresentou-o à diretoria médica, formulou fluxograma de rotina do serviço e responsabilidade da equipe multidisciplinar e elaborou folder educativo. Deu-se início a aplicação dos testes nos recém-nascidos da maternidade, divulgação e feedback à equipe assistencial. Os testes com escore duvidoso são encaminhados para reteste, na própria maternidade, e escores alterados encaminhados ao serviço de odontologia do hospital para frenotomia. Resultados: A adesão dos pediatras, a satisfação dos pais com o diagnóstico precoce da anquiloglossia e a curva crescente de testes realizados, minimizando a possível interferência do frênulo lingual nas funções estomatognáticas e desmame precoce da amamentação.
Conclusão: O trabalho realizado pela Fonoaudiologia, trouxe clareza à equipe médica sobre o objetivo e importância do teste; agregou valor à maternidade e, repercutiu positivamente no diagnóstico precoce da anquiloglossia e seus impactos.

Abstract:
Introduction: The Protocol of Evaluation of the Tongue Frenulum in babies makes it possible to standardize the anatomical finding of the frenulum and its impact on the functions of suction and swallowing during breastfeeding. It served as a basis for Law No. 13,002 of June 20, 2014, published in the Journal ‘Diário Oficial’, making it mandatory to perform it out in maternity wards in the country, as an incident part of Neonatal Screening. Thus, the speech therapists were trained and implemented, in 2015, the “Tongue Test”, making it a pioneer and a reference in the performance of this service. Objective: To describe the implementation of the “Tongue Test” in a maternity hospital n Recife. Method: Descriptive experience report on the implementation of the “Tongue Test” in a private maternity hospital. The speech therapy team structured the project, presented it to the medical director, formulated a routine flowchart, appointed responsibility to every member of the multidisciplinary crew and prepared an educational folder. The application of the test on newborns started, as well as the dissemination and feedback to the care team. Tests with doubtful scores receives instructions to return after 30 days or less, for a reevaluation, at the maternity hospital itself, and altered scores are referred to the dental service for a release of the lingual frenulum procedure. Results: Acceptance by pediatricians, parents’ satisfaction about the early diagnosis of tongue-tie and the growing curve of the tests performed, minimizing the possible interference of the lingual frenulum in stomatognathic functions and early weaning from breastfeeding. Conclusion: The work performed by the Speech Therapy team, brought knowledge to the medical team about the purpose and importance of the test; increased financial gain to the medical institution and had a positive impact on the early diagnosis of tongue-tie and its impacts.
