CUIDADO FARMACÊUTICO COMO CONTRIBUIÇÃO PARA O FORTALECIMENTO DA ATENÇÃO PRIMÁRIA À SAÚDE
Atualmente, destaca-se a importância da equipe multiprofissional para o cuidado do paciente, enfatizando o cuidado farmacêutico, que constitui a ação integrada do mesmo com a equipe de saúde, centrada no paciente, focando no alcance de resultados terapêuticos concretos. O objetivo deste trabalho foi apresentar a experiência da implantação do Cuidado Farmacêutico em uma unidade de Atenção Primária à Saúde, no município de Ponta Grossa, Paraná. Foi realizado um estudo transversal, por meio da análise documental dos prontuários farmacêuticos dos pacientes atendidos pelo Serviço de Farmácia de uma UBS, em parceria com o projeto de extensão “Educação em Saúde” no município de Ponta Grossa, Paraná, no período de fevereiro de 2018 a maio de 2019. A equipe realizou consultas farmacêuticas dos tipos: a) visita em domicílio, em que foram realizadas 50 visitas a pacientes com diabetes insulinizados, a fim de avaliar questões relacionadas à insulina. Verificou-se que 58% armazenavam de forma incorreta e 36% realizavam incorretamente a homogeneização da insulina de ação intermediária. O atendimento domiciliar possibilitou a compreensão da realidade do paciente, viabilizando que as intervenções farmacêuticas fossem adaptadas às realidades individuais; b) entrega de aparelho glicosímetro por meio de consulta na unidade de saúde a 80 pacientes, contribuindo para otimizar o uso dos insumos e o empoderamento do paciente; c) avaliação clínica a fim de contribuir para o manejo da doença a 13 pacientes, com a detecção de 81 problemas relacionados à farmacoterapia e realização de 123 intervenções farmacêuticas. Este estudo demonstra a importância da inserção do Cuidado Farmacêutico nas equipes multidisciplinares de atenção primária à saúde, além identificar algumas necessidades dos pacientes do sistema, como a realização de exames e revisão da farmacoterapia.

Abstract:
Currently, the importance of the multidisciplinary team for patient care is highlighted, emphasizing pharmaceutical care, which constitutes an integrated action of the same with the health team, centered on the patient, focusing on achieving concrete therapeutic results. The objective of this study was to present the experience of implementing Pharmaceutical Care in a Primary Health Care unit, in the municipality of Ponta Grossa, Parana. A cross-sectional study was carried out, by means of documentary analysis of the pharmaceutical records of patients treated by the Pharmacy Service of a BHU, in partnership with the extension project "Education in Health" in the municipality of Ponta Grossa, Parana, in the period of February 2018 to May 2019. The team held pharmaceutical consultations of the following types: a) home visits, in which 50 visits were made to patients with insulinized diabetes, in order to assess issues related to insulin. It was found that 58% stored incorrectly and 36% incorrectly performed the homogenization of intermediate-acting insulin.Home care enabled the understanding of the patient's reality, enabling pharmaceutical interventions to be adapted to individual realities; b) delivery of a glucometer through consultation at the health unit to 80 patients, contributing to optimize the use of inputs and the empowerment of the patient; c) clinical evaluation in order to contribute to the management of the disease in 13 patients, with the detection of 81 problems related to pharmacotherapy and the performance of 123 pharmaceutical interventions. This study demonstrates the importance of inserting Pharmaceutical Care in multidisciplinary teams of primary health care, in addition to identifying some needs of patients in the system, such as exams and pharmacotherapy reviews.
