Abstract: Cutaneous cryptococcosis usually develops from hematogenous dissemination in immunocompromised hosts and is associated with disseminated infection. There is a clinical condition, recognized as primary cutaneous cryptococcosis (PCC), restricted to the skin and rarely reported in immunocompetent individuals. We present a case of an elderly, immunocompetent male, who developed edema in his hand, associated with pain and purulent exudation, which began after skin trauma caused by handling a cactus. Histopathological evaluation showed findings suggestive of cutaneous cryptococcosis, which was confirmed by culture of the tissue fragment, which identified Cryptococcus gattii. Although rare and with a generally favorable prognosis, primary cutaneous cryptococcosis should be included in the differential diagnosis of skin and soft tissue infections.
