CIRURGIA DE AUMENTO DE COROA CLÍNICA ESTÉTICA EM ELEMENTO UNITÁRIO COM FINALIDADE DE TRATAMENTO DO SORRISO GENGIVAL: RELATO DE CASO
Uma importante fração das expressões faciais é obtida no terço inferior da face através do sorriso. Em alguns casos, o paciente apresenta uma desproporção entre a estética rosa e branca, resultando em um sorriso gengival. Sua etiologia pode englobar: crescimento vertical excessivo maxilar, extrusão dentoalveolar, lábio superior curto ou a hiperatividade do mesmo, erupção passiva e/ou ativa alterada e hiperplasia gengival. O tratamento é obtido através de um correto diagnóstico e pode envolver diversos procedimentos odontológicos, dentre eles, a cirurgia de aumento de coroa clínica com finalidade estética está indicada quando os dentes anteriores são curtos ou tem exposição excessiva de tecido gengival e quando o contorno gengival é irregular e seu propósito é estabelecer uma relação adequada na posição da margem gengival com o lábio e aumentar a coroa clínica dos dentes, proporcionando harmonia estética entre altura e largura dos elementos anteriores. O objetivo desse trabalho é relatar um caso clínico de um paciente de 20 anos que relatou insatisfação com seu sorriso, uma vez que o elemento 22 apresentava coroa clínica curta descrevendo a técnica cirúrgica de aumento de coroa clínica estética através de incisões reta sem angulações e intrasulcular, remoção do colar gengival, retalho total e osteotomia, a fim de tratar a queixa estética relatada pelo mesmo. Foram feitos pós-operatórios de 1, 3 e 12 meses para acompanhamento do caso. O paciente relatou estar muito satisfeito, pois foi obtido estabilidade dos resultados alcançados, graças ao diagnóstico, planejamento e tratamento adequados e um acompanhamento rigoroso durante o período de 1 ano.

Abstract:
An important fraction of facial expressions is obtained in the lower third of the face through a smile. In some cases, the patient has a disproportion between the pink and white aesthetic, resulting in a gingival smile. Its etiology may include: excessive vertical maxillary growth, dentoalveolar extrusion, short upper lip or its hyperactivity, altered passive and / or active eruption and gingival hyperplasia. The treatment is obtained through a correct diagnosis and can involve several dental procedures, among them, the surgical crown lenghtening with aesthetic purpose is indicated when the anterior teeth are short or have excessive exposure of gingival tissue and when the gingival contour is irregular and its purpose is to establish an adequate relationship in the position of the gingival margin with the lip and to increase the clinical crown of the teeth, providing aesthetic harmony between the height and width of the anterior elements. The aim of this study was to report a clinical case of a 20-year-old patient who reported dissatisfaction with his smile, since element 10 presented a short clinical crown describing the surgical technique of increasing the aesthetic clinical crown through straight incisions without angulations and intrasulcular, removal of the gingival collar, total flap and osteotomy, in order to treat the aesthetic complaint reported by the same. Postoperative periods of 1, 3 and 12 months were performed to monitor the case. The patient reported being very satisfied, as stability of the results achieved was obtained, thanks to the proper diagnosis, planning and treatment and strict monitoring during the period of 1 year.
