ASSISTÊNCIA DE ENFERMAGEM A PACIENTES COM HEPATITE C EM HEMODIÁLISE: RELATO DE EXPERIÊNCIA
Pacientes com doença renal terminal (DRCT) em hemodiálise apresentam risco maior de infecção pelo vírus da hepatite C (HCV) quando comparados à população em geral, devido à frequente exposição sanguínea e infecções nosocomiais. Este estudo foi desenvolvido com o objetivo de relatar os cuidados, as precauções e os preparos necessários aos profissionais de enfermagem voltados à rotina de pacientes dialíticos, de forma a evitar a contaminação e propagação do vírus da hepatite C (HCV) entre pessoas em hemodiálise acometidas por insuficiência renal crônica (IRC). Trata-se de um estudo descritivo do tipo relato de experiência, realizado no primeiro trimestre do ano de 2019 em um hospital do município de Fortaleza. O rastreio para hepatite C deve ser realizado em todos os pacientes de terapia renal substitutiva. Caso o teste sorológico seja negativo, o paciente segue a rotina de realizar sorologias semestralmente. Preconiza-se que os pacientes em HD com hepatite C estejam em salas separadas para evitar a transmissão do vírus e os riscos de contaminação horizontal pelo HCV. Entretanto, caso o paciente possua mais de um tipo de hepatite, este deverá dialisar individualmente à beira do leito. A rigorosa limpeza e desinfecção da máquina de HD associada à restrição da diálise de pessoas com hepatite C em turno diferente evidenciou-se como um método eficaz no controle da transmissão do HCV entre diálises. Exames admissionais e pós-admissionais são essenciais para a avaliação completa e o conhecimento acerca do estado sorológico do paciente, para a implementação da devida assistência de enfermagem durante a hemodiálise. A implantação dessas medidas na organização assistencial de enfermagem para com pacientes em hemodiálise demonstrou-se um método eficaz para a redução dos níveis de infecção e transmissão do HCV.

Abstract:
Patients with end-stage renal disease (ESRD) on hemodialysis have a higher risk of infection with the hepatitis C virus (HCV) when compared to the general population, due to the frequency of blood exposure and nosocomial diseases. This study was developed with the aim of relating the care, precautions and preparations to nursing professionals focused on the routine of dialysis patients, in order to avoid the contamination and spread of the hepatitis C virus (HCV) among people on hemodialysis affected by chronic renal failure (CRF). This is a descriptive study of the type of experience report, carried out in the first quarter of 2019 in a hospital in the city of Fortaleza. Screening for hepatitis C should be performed in all renal replacement therapy patients. If the serological test is negative, the patient follows the routine of performing serological tests every six months. It is recommended that patients on HD with hepatitis C chosen in separate rooms to avoid transmission of the virus and the risks of horizontal contamination by HCV. However, if the patient has more than one type of hepatitis, he must dialysis individual at the bedside. The rigorous cleaning and disinfection of the HD machine associated with the restriction of dialysis in people with hepatitis C in a different shift has proved to be an effective method to control HCV transmission between dialysis. Admission and post-admission examinations are essential for the complete assessment and knowledge about the patient's serological status, for the implementation of the appropriate nursing care during hemodialysis. The implementation of these measures in the nursing care organization for hemodialysis patients is an effective method for reducing the levels of HCV infection and transmission.
