APLICAÇÃO DE PAPAÍNA EM PÓ EM DEISCÊNCIA DE FERIDA OPERATÓRIA INFECTADA
Com o avanço da tecnologia na
área da saúde, novas terapias tópicas vêm
sendo incorporadas na prevenção e tratamento
de feridas. Entre estas, a papaína se destaca
por seu custo e benefício científico com
ação desbridante enzimática, bactericida,
bacteriostático, anti-inflamatório, estimula a
força tênsil, e alinhamento das fibras de colágeno
para obtenção de cicatrização uniforme.
Pode ser utilizada em lesões com diferentes
etiologias, faixas etárias e fases do processo
de cicatrização. Objetivo: avaliar a aplicação
da papaína em pó em deiscência de ferida
operatória infectada. Método: Foram realizadas
avaliação e evolução dos casos em unidade
cirúrgica, em um hospital universitário do Rio de
Janeiro, julho a agosto de 2017, acompanhados
de registro fotográfico respeitando os aspectos
éticos e legais da pesquisa. Considerando a
necessidade de desbridamento e de estimular
o crescimento de tecido de granulação, optouse pelo uso da papaína em pó em diferentes
concentrações associada ao soro fisiológico
nas lesões. As lesões apresentaram diminuição
gradativa do tecido desvitalizado, aumento de
tecido de granulação e contração de bordas. As
pacientes foram acompanhadas até epitelização
completa, sem necessidade de reintervenção
cirúrgica. Resultado: O processo cicatricial ocorreu de forma plena com o tratamento tópico atribuído à atuação sistematizada
e especializada do enfermeiro. Conclusão: A avaliação do enfermeiro associada a
escolha da cobertura tópica adequada foi importante no tratamento, considerando
efetividade e menor custo na epitelização da deiscência da ferida operatória infectada.
Ressalta-se a necessidade de novos estudos para divulgar o conhecimento sobre o
uso da papaína em feridas infectadas

Abstract:
With the advancement of technology in the area of health, new topical
therapies have been incorporated into the prevention and treatment of wounds. Among
these, papain stands out for its cost and scientific benefit with enzymatic, bactericidal,
bacteriostatic, anti-inflammatory, degrading action, stimulates tensile strength, and
alignment of collagen fibers to achieve uniform healing. It can be used in lesions
with different etiologies, age groups and stages of the healing process. Purpose: to
evaluate the application of powdered papain in infected operative wound dehiscence.
Method: The evaluation and evolution of the cases were carried out in a surgical unit,
in a university hospital in Rio de Janeiro, from July to August 2017, accompanied
by a photographic record respecting the ethical and legal aspects of the research.
Considering the need for debridement and to stimulate the growth of granulation tissue,
it was decided to use powdered papain in different concentrations associated with
saline solution in the lesions. The lesions showed a gradual decrease of devitalized
tissue, increase of granulation tissue and contraction of borders. The patients were
followed up until complete epithelization, without the need for surgical reintervention.
Result: The cicatricial process occurred in full with topical treatment attributed to the
systematized and specialized performance of the nurse. Conclusion: The evaluation
of the nurse associated with the choice of adequate topical coverage was important
in the treatment, considering effectiveness and lower cost in the epithelization of the
dehiscence of the infected operative wound. The need for further studies to disseminate
knowledge about the use of papain in infected wounds is emphasized.
