ACOMPANHAMENTO FARMACOTERAPÊUTICO DE PACIENTES IDOSOS ATENDIDOS EM UMA UNIDADE BÁSICA NO MUNICÍPIO DE SAÚDE DE MISSÃO VELHA – CEARÁ
O estudo tem como objetivo descrever o acompanhamento farmacoterapêutico em pacientes idosos com Diabetes Mellitus, em uma Unidade Básica de Saúde (UBS) no interior de Missão Velha – CE, sendo submetido e aprovado no comitê de ética e pesquisa da Faculdade de Juazeiro do Norte, sob o número de parecer 3.623.456. O acompanhamento farmacoterapêutico foi realizado em um grupo de 40 idosos, com predomínio de pacientes do sexo feminino (55%), e idade média de 70 anos, no qual identificou-se problemas ligados aos medicamentos. Diante destes, foram implementadas intervenções farmacêuticas visando dirimir erros na farmacoterapia do paciente, relacionadas à adesão terapêutica, ineficácia terapêutica e interações medicamentosas. O acompanhamento por parte de um profissional farmacêutico torna-se fundamental, orientando de forma especial estes pacientes e buscando conscientizá-los sobre a importância da adesão à terapia medicamentosa na melhora da sua saúde.

Abstract:
The study aims to describe the pharmacotherapeutic follow-up in elderly patients with Diabetes Mellitus, in a Basic Health Unit (UBS) in the interior of Missão Velha - CE, being submitted and approved by the ethics and research committee of the Faculty of Juazeiro do Norte, under opinion number 3,623,456. In the pharmacotherapeutic follow-up performed in 40 elderly, it was observed that there was a predominance of female patients (55%), with an average age of 70 years, where drug-related problems were identified, such as: need, effectiveness and safety. Given the problems encountered, pharmaceutical interventions were performed with the objective of solving problems in the patient's Pharmacotherapy, being the interventions related to therapeutic adherence, therapeutic ineffectiveness and adverse reaction to medications. Therefore, it is concluded that the monitoring by a pharmaceutical professional is indispensable, promoting special attention to the patients' education, seeking to make them aware of their health condition and the importance of adherence to drug therapy.
