ABORDAGEM DA MULHER HOMOSSEXUAL E BISSEXUAL NA CONSULTA GINECOLÓGICA
A população LGBT aos poucos tem seu espaço reconhecido, contudo, no Brasil, observa-se uma defasagem na identificação de mulheres lésbicas e bissexuais (LB). Dentro da esfera da saúde tem-se a baixa adesão a consultas ginecológicas, por experiências prévias negativas, pelo foco na contracepção e gravidez ou pela falta de conhecimento dos profissionais sobre as necessidades específicas dessa população. Já do ponto de vista do médico, são diversos mitos e estigmas que rodeiam o assunto e geram dúvidas e receio na investigação da saúde de mulheres LB. No entanto, são observadas altas taxas que cursam diretamente com a negligência na prevenção e diagnóstico de ISTs dentre mulheres LB. Os índices de infecção por HPV chegam a mais de 30% e 10% das mulheres disseram não precisar de citológico porque não se relacionavam com homens, sendo que destes 10%, 89% foram orientadas por profissionais de saúde a não colher exame citológico. Dessa forma, infecções como HIV, HPV, sífilis e outras doenças são prevalentes, o que revela alta vulnerabilidade, acentuada por depressão, obesidade e abuso de drogas. Assim, a anamnese, as condutas e as orientações, devem considerar as particularidades de cada parcela da população. O exame físico ginecológico e as vacinas devem ser iguais, independente da orientação sexual, havendo algumas singularidades. Por fim, é importante que todos os profissionais que lidam diretamente com essas pacientes estejam treinados para acolher, esclarecer e indicar os procedimentos necessários, visando um atendimento integrado e de qualidade. Formulários médicos inclusivistas e propagandas em saúde voltadas às demandas LB, são formas de alcançar esses objetivos.

Abstract:
In recent years, the LGBT community has gradually been recognized. However, in Brazil, there is a gap in the identification of lesbians and bissexual (LB) women. In health, there is a low adherence to gynecological consultations due to many reasons. To begin with, previous negative experiences carry a stigma in the medical world. In addition, there is a strong focus on contraception and pregnancy. Finally, there is a lack of knowledge from professionals about the specific needs of this population. From the doctor's point of view, there are several misconceptions that surround the subject. These misconceptions generate doubts and fear within the investigation of LB women's health. However, high rates that directly lead to negligence in the prevention and diagnosis of STI among LB women are observed. Within this population, HPV infection rates reach more than 30%, of which 10% of the women say they did not need cytology because they did not have a relationship with men. From the 10% of these LB women, 89% of them were instructed by health professionals not to have a cytological exam. Thus, infections such as HIV, HPV, syphilis and other diseases are prevalent. As a result, these women experience high vulnerability, depression, obesity and drug abuse. Therefore, anamnesis, conduct and guidelines, must consider the particularities of each part of the population. With some singularities, gynecological physical examination and vaccines must be the same regardless of sexual orientation. Finally, it is important that all professionals who deal directly with these patients are trained to welcome, clarify and indicate the necessary procedures. The medical world then would aim to provide an integrated and quality service. Inclusive medical forms and health advertisements geared to LB demands are ways of achieving these goals.
