Abstract: The present study seeks to address how spirituality has been recognized as a relevant component in the recovery of critically ill patients in Intensive Care Units (ICU), considering in the specific literature different evidence that highlight its influence on biopsychosocial mechanisms. The main objective of this study is to analyze, from the perspective of medicine, the influence of spirituality on the recovery process of these patients. Studies show that spiritual practices act on the hypothalamic-pituitary-adrenal (HPA) axis, significantly reducing cortisol levels and attenuating responses to acute stress, causing synergistic effects in reducing the need for sedatives and analgesia. In addition, an inverse correlation is observed between spiritual engagement and pro-inflammatory cytokine concentration indices (IL-6, TNF-α), indicating modulation of the immune response. In the cardiovascular context, there is an improvement in heart rate variability (HRV), when using practices such as mindfulness, and with this there is a reduction in the risk of arrhythmias. It is also important to consider that spirituality acts as a protective factor against adaptive disorders, with a considerable decrease in anxiety and depression scores on validated scales (HADS). However, there must be a clinical integration that requires structured protocols, in order to avoid negative religious coping. As it is a literature review, the strategy used to search for materials relevant to the subject was the use of databases available in the electronic libraries SciElo, Lilacs, Medline and Pubmed, considering more recent scientific productions. In this bibliographic survey carried out on the internet, descriptors based on the Health Sciences Descriptors were used. It is concluded that the incorporation of spirituality into intensive protocols can humanize care and optimize prognosis, thus requiring the promotion of future consistent clinical trials to consolidate its role in current medicine.
