A IMPORTÂNCIA DA VIVENCIA DO ESTUDANTE DE MEDICINA NO ACOMPANHAMENTO PRÉ NATAL DE GESTANTE COM POSSÍVEL MIOCARDIOPATIA PERIPARTO EM GESTAÇÃO ANTERIOR
INTRODUÇÃO: Seguindo as Diretrizes Curriculares do Curso de Medicina Da Atenção às Necessidades Individuais de Saúde, mostra-se necessário o aprendizado prático e teórico da medicina, antes de sua efetivação, para que haja uma postura ética na relação médico paciente, buscando informá-lo e esclarecê-lo. OBJETIVO: Correlacionar a importância do atendimento da Atenção Primária à Saúde, principalmente na área de Obstetrícia, com o possível diagnóstico prévio de Miocardiopatia Periparto e seu diagnóstico diferencial com a Eclâmpsia. RELATO DE EXPERIÊNCIA: A experiência ocorreu a partir do acompanhamento de uma consulta de pré natal, em uma unidade básica de saúde, na cidade de Franca, SP. A paciente em questão, estava na sua terceira gestação e encontrava-se apreensiva com a gravidez e a saúde de seu filho que iria nascer devido ao seu mau passado obstétrico e a suspeita diagnóstica de miocardiopatia periparto, transcorrida durante seu primeiro puerpério. No entanto, a paciente apresentava um acompanhamento de pré natal irregular e dados incompletos em seu prontuário. Dessa forma, foram realizadas anamnese completa, coleta de dados da história pregressa pessoal e obstétrica, exame físico e encaminhamento para o cardiologista, a fim de esclarecer sobre o provável diagnóstico de miocardiopatia periparto e orientações sobre o pré-natal. Ao final da consulta a paciente encontrou-se tranquila e orientada, e os alunos tiveram como proveito a discussão teórico prática sobre a doença e seus diagnósticos diferenciais e além do funcionamento da atenção primária à saúde. CONCLUSÃO: O presente estudo apresentou a análise de um relato de experiência vivenciado em uma Unidade Básica de Saúde de Franca, SP, e teve como enfoque a discussão da importância do atendimento na Atenção Primária à Saúde, a relação médico-paciente e sua atuação no diagnóstico prévio da mesma, relacionando a prática ao conhecimento teórico.

Abstract:
INTRODUCTION: Following the Curricular Guidelines of the Medicine Course ‘Attention to Individual Health Needs’, it is necessary to have a practical and theoretical learning of medicine, before its implementation, so that there is an ethical posture in the doctor-patient relationship, which seeks to inform and to enlighten her. OBJECTIVE: To correlate the importance of appointment in the Primary Health Care Attention, especially in the area of Obstetrics, with the possible previous diagnosis of Peripartum Cardiomyopathy and its differential diagnosis with Eclampsia. EXPERIENCE REPORT: The experience occurred from the follow-up of a prenatal appointment, in a basic health unit, in the city of Franca, SP. The patient in question was in her third pregnancy and was apprehensive about the pregnancy and the health of her unborn child due to her poor obstetric past and the suspected diagnosis of peripartum cardiomyopathy, which occurred during her first puerperium. However, the patient had irregular prenatal follow-up and incomplete data in her medical record. Thus, a complete anamnesis, data collection of past personal and obstetric history, physical examination and referral to the cardiologist were performed in order to clarify the probable diagnosis of peripartum cardiomyopathy and guidance on prenatal care. At the end of the appointment, the patient was calm and informed, and the students benefited from the theoretical and practical discussion about the disease and its differential diagnoses and beyond the functioning of primary health care attention. CONCLUSION: The present study presented the analysis of an experience report occurred in a Basic Health Unit in Franca, SP, and focused on the discussion of the importance of the appointment in the Primary Health Care Attention, the doctor-patient relationship and its performance in the previous diagnosis, relating the practice to theoretical knowledge.
