Abstract: Orbital xanthoma (XO) is a rare manifestation characterized by the accumulation of lipid-filled macrophages in orbital tissues. This article presents the case of a 53-year-old man with no known lipid comorbidities who developed an expansive orbital lesion. A CT scan revealed a mass in the lateral wall of the left frontal sinus with ocular proptosis, and surgical resection was performed. Histopathological analysis confirmed the diagnosis of XO, with the presence of xanthomatous macrophages and multinucleated giant cells. The case highlights the possibility of XO developing in patients without obvious dyslipidemia, suggesting that inflammatory processes or immune dysfunctions may contribute to the pathogenesis.
