Abstract: Early menarche, defined as the onset of menstruation before the age of eight, is a multifactorial event with significant implications for adolescent physical and mental health. This systematic review aimed to identify and analyze the main risk factors associated with early menarche. Searches were conducted in PubMed, LILACS, and Cochrane Library, including studies published in the last ten years in English, Portuguese, and Spanish. Following PRISMA guidelines, six studies were included in the qualitative synthesis. The main risk factors identified were childhood obesity, exposure to endocrine-disrupting chemicals, genetic predisposition, psychosocial stress, and inadequate dietary patterns. Obesity emerged as the most consistent predictor, since increased body mass index is associated with hormonal changes that trigger early puberty. Environmental exposures and psychosocial adversities also play relevant roles. In conclusion, early menarche requires multidisciplinary attention focused on promoting healthy lifestyle habits, reducing exposure to chemical substances, and providing psychosocial support. This is a relevant issue for public health, highlighting the importance of preventive policies and early clinical monitoring.
