Abstract: Sarcopenia is a geriatric syndrome characterized by the progressive loss of skeletal muscle mass and strength, associated with aging and clinical conditions, which increases the risk of falls, hospitalizations, and mortality. The SARC-Calf, a screening tool that combines the SARC-F questionnaire with calf circumference measurement, has emerged as a practical, low- cost, and accurate method for identifying sarcopenia in older adults, especially in primary health care. This study consists of an integrative literature review, conducted between July and October 2025, in PubMed, SciELO, Google Scholar, and BVS databases, including original articles published between 2021 and 2025. After applying inclusion and exclusion criteria, four studies comprised the final sample. Findings indicate that SARC-Calf presents sensitivity ranging from 60% to 95% and higher specificity compared to SARC-F alone, as well as a significant association with falls, hospitalizations, and frailty in older adults. The discussion highlights that SARC-Calf is a valuable tool for clinical practice and public health, being useful both for early screening and for risk stratification in community-dwelling older adults. However, challenges remain, such as the lack of universal cut-off points for calf circumference and the scarcity of large-scale longitudinal studies. In conclusion, SARC-Calf represents a highly applicable resource for sarcopenia screening and fall prevention among older adults in primary care, providing support for clinical decision-making, public health strategies, and future research on healthy aging.
