Abstract: Allergic purpura, or immunoglobulin A vasculitis, is the most frequent systemic vasculitis in childhood, characterized mainly by hemorrhagic skin manifestations, often associated with joint, gastrointestinal symptoms and, in some cases, renal impairment. Although it has a generally self-limited course, early recognition is essential for the monitoring and prevention of complications, especially renal. The diagnosis is predominantly based on clinical criteria, being complemented by laboratory tests and imaging methods when indicated. However, in contexts of social vulnerability, structural barriers such as limited access to exams, difficulty in referring to specialized services and weaknesses in Primary Health Care contribute to diagnostic delays and inadequate follow-up. In this article, allergic purpura in childhood is discussed based on its clinical manifestations and diagnostic criteria, highlighting palpable purple as a sensitive marker of inequalities in access to diagnosis and health care for children in socially vulnerable territories.
