VIVÊNCIA DA EQUIPE MULTIDISCIPLINAR FRENTE A UMA CIRURGIA DE ALTA COMPLEXIDADE - CIRURGIA CITORREDUTORA COM HIPEC
Foi realizada uma pesquisa descritiva, de natureza qualitativa, na forma de relato de experiência, paciente submetida a Cirurgia citorredutora com HIPEC, realizada em hospital oncológico de Campo Grande/MS, em novembro de 2017. Foi a quarta cirurgia da história de Mato Grosso do Sul, paciente do sexo feminino, 32 anos. É um procedimento de alta complexidade, exige o manejo multiprofissional no pré e pós-operatório para boa evolução e recuperação. Delimitou-se estratégias no cuidado multiprofissional em todos os processos envolvidos. Na cirurgia em questão a abordagem foi em um caso de Mesotelioma Peritoneal Maligno. No pré-operatório foi realizado preparo nutricional com dietoterapia para suporte calórico, proteico e cicatrização, além de preparo fisioterápico de fortalecimento muscular e respiratório. Durante a cirurgia atentou-se para controle rigoroso na ressecção de todos os tumores visíveis e no procedimento quimioterápico, além de apurada monitorização anestésica. Após o procedimento a paciente foi encaminhada extubada e estável para Unidade de Terapia Intensiva com atenção especial à hidratação vigorosa, controle de dor através bomba da PCA via cateter peridural e controle rigoroso de eletrólitos. A paciente apresentou evolução satisfatória e controle clínico dos sinais e sintomas. A fisioterapia realizou controle da capacidade funcional e ventilatória através da ventilação não invasiva (reversão de atelectasia) e mobilização precoce. A terapia nutricional iniciou com uso de nutrição parenteral e evoluiu gradativamente até o uso exclusivo da via convencional. A enfermagem atentou-se aos dispositivos e curativos, além da promoção do autocuidado. Após período de vigência clínica operatória a paciente recebeu alta com bom prognóstico. Tratando-se de um procedimento complexo e inédito, a cirurgia exigiu preparo técnico-científico de todos os profissionais e a atuação com entrosamento da equipe multidisciplinar foi fundamental para promover cuidado integral a paciente.

Abstract:
Descriptive and qualitative research was carried out, in the form of a case study, using a patient who underwent Cytoreductive surgery with HIPEC. This study was performed at Cancer Hospital in Campo Grande, State of Mato Grosso do Sul (MS), Brazil, in November 2017. It was the fourth surgery in the history of the State of MS, a female patient, 32 years old. It is a highly complex procedure, requiring multi-professional management in the pre and postoperative period for good evolution and recovery. Strategies were defined in multi-professional care in all the steps and processes involved. In the surgery in question, the approach was in a case of Malignant Peritoneal Mesothelioma. In the preoperative period, nutritional preparation with diet therapy was carried out for caloric, protein, and healing support. Additionally, physiotherapy preparation for muscle and respiratory strengthening was performed. During surgery, strict control was taken in the resection of all visible tumors and the chemotherapy procedure, in addition to accurate anesthetic monitoring. After the procedure, the patient was referred extubated and stable to the Intensive Care Unit with special attention to vigorous hydration, pain control through the PCA pump via an epidural catheter, and strict electrolyte control. The patient presented a satisfactory evolution and clinical control of signs and symptoms. Physical therapy performed control of functional and ventilatory capacity through noninvasive ventilation (atelectasis reversal) and early mobilization. Nutritional therapy started with the use of parenteral nutrition and progressed gradually to the exclusive use of the conventional route. The Nursing team paid attention to devices and dressings, in addition to promoting self-care. After the operative clinical period, the patient was discharged with a good prognosis. As this is a complex and unprecedented procedure, the surgery required technical and scientific preparation from all professionals and the work with the multidisciplinary team was essential to promote comprehensive patient care.
