Abstract: Prostate cancer is a disease of great importance for public health, with a high incidence and variable prognosis. The most common type of prostate cancer is acinar adenocarcinoma, which represents approximately 90% of cases. However, its histological presentation is variable and may show characteristics of benign lesions, other types of tumors or structures adjacent to the prostate, thus making the diagnosis of the disease difficult. Histological variants can also present significant prognostic differences and resistance to certain treatments, configuring extremely important diagnostic findings for therapeutic decision-making. It is extremely important that the pathologist is attentive to histological variations in the prostate to ensure assertiveness in the diagnosis, and, based on the findings present in the tissue and recognizing their clinical significance, assist in the best choice of treatment for each patient. In this chapter, we address and discuss the main characteristics of some of the variants of prostate acinar adenocarcinoma, some useful tools to identify them and their prognostic significance.
