VARIAÇÕES ANATÔMICAS DO PLEXO BRAQUIAL E SUAS POSSÍVEIS IMPLICAÇÕES CLÍNICAS E CIRURGICAS
Introdução: O plexo braquial e suas ramificações têm sido objeto de muitos estudos anatômicos devido à incidência relativamente frequente de variações. Certas variações do plexo braquial causam disseminação imprevisível do anestésico local ou, às vezes, distribuição indesejada do bloqueio. O conhecimento prévio das variações existentes auxilia os profissionais da saúde no decorrer dos procedimentos neurocirúrgicos. Objetivo: Analisar as variações anatômicas no plexo braquial em humanos e as suas implicações clínicas e cirúrgicas. Metodologia: Trata-se de uma revisão sistemática de artigos indexados nas bases de dados SciELO, PubMed, LILACS, SPRINGERLINK, SCIENCE DIRECT e LATINDEX, no período de março a abril de 2022. Foram incluídos artigos originais envolvendo variações anatômicas do plexo braquial em humanos. A coleta foi realizada por dois revisores independentes e a análise de concordância interobservador foi observada por meio do teste de Kappa (Bioestat V 5.0). Resultados: Ao final da busca foram selecionados 11 artigos, caracterizados quanto à amostra, método para avaliar a estrutura anatômica e principais resultados. Onze estudos (100%) utilizaram o método de dissecação de cadáveres. Quatro (36.36%) dos estudos observaram uma comunicação entre o nervo musculocutâneo e mediano. Um (9.09%) estudo observou o nervo mediano por 3 raízes. Um (9.09%) estudo relatou o tronco superior em frente ao escaleno anterior, outro (9.09%) relatou ainda a ausência do nervo musculocutâneo. Conclusão: Observou-se que as variações anatômicas do plexo braquial não são achados incomuns. A variação mais prevalente nesta revisão foi a presença de comunicação entre o nervo mediano e musculocutâneo. O conhecimento dessas variações tende a reduzir o risco dos procedimentos cirúrgicos, sobretudo em neurocirurgias eletivas.

Abstract:
Introduction: The brachial plexus and its ramifications have been the subject of many anatomical studies due to the relatively frequent incidence of variations. Certain variations of the brachial plexus cause unpredictable spread of local anesthetic or, sometimes, undesired distribution of the block. Prior knowledge of the existing variations helps health professionals during neurosurgical procedures. Objective: To analyze the anatomical variations in the brachial plexus in humans and their clinical and surgical implications. Methodology: This is a systematic review of articles indexed in the SciELO, PubMed, LILACS, SPRINGERLINK, SCIENCE DIRECT and LATINDEX databases, from March to April 2022. Original articles involving anatomical variations of the brachial plexus in humans were included. . Data collection was performed by two independent reviewers and interobserver agreement analysis was performed using the Kappa test (Bioestat V 5.0). Results: At the end of the search, 11 articles were selected, characterized as to the sample, method to evaluate the anatomical structure and main results. Eleven studies (100%) used the cadaver dissection method. Four (36.36%) of the studies observed communication between the musculocutaneous and median nerves. One (9.09%) study looked at the median nerve through 3 roots. One (9.09%) study reported the upper trunk in front of the scalene anterior, another (9.09%) also reported the absence of the musculocutaneous nerve. Conclusion: It was observed that anatomical variations of the brachial plexus are not uncommon findings. The most prevalent variation in this review was the presence of communication between the median and musculocutaneous nerves. Knowledge of these variations tends to reduce the risk of surgical procedures, especially in elective neurosurgeries.
