Abstract: Behcet's Syndrome is a chronic inflammatory vasculitis with variable manifestations, often refractory to conventional treatment. The diagnosis is based on the International Study Group for Behçet Disease criteria, and the etiology is still unknown, but it is known that genetic and environmental factors are involved. Its most prevalent manifestation is Behcet's uveitis, which is one of the main causes of blindness worldwide. Vascular damage is caused by sustained inflammation and immune system hyperfunction. Conventional therapy is carried out with steroids and immunomodulators, but some patients are resistant to this therapy and relapse. The objective of this work is to evaluate the efficacy of Infliximab in the treatment of refractory manifestations and analyzed 15 studies selected from Pubmed and Virtual Health Library, including observational studies and controlled clinical trial published in the last 10 years in Portuguese and English. As a result, all selected studies reported symptomatic improvement with complete or partial remission of the ocular, vascular, intestinal, neurological, skin, joint and cardiovascular manifestations of the disease. The improvement of manifestations after the use of Infliximab is due to this drug acting directly in the pathogenesis of inflammation, blocking TNF receptors and promoting the increase of CD4+ cells and IL-10 expression, reducing chronic inflammation, enabling clinical improvement of refractory manifestations.
