ÚLCERA TERMINAL DE KENNEDY: UMA REVISÃO INTEGRATIVA DE LITERATURA
A Úlcera Terminal de Kennedy (UTK) é definida como um tipo particular de lesão desenvolvida durante o processo de morte. Diante do exposto, o objetivo deste estudo é avaliar a produção científica acerca da UTK, pontuando suas características. O presente estudo tratou-se de uma revisão integrativa de literatura realizada através do levantamento bibliográfico na base de dados MEDLINE, PUBMED, Google Acadêmico. Foram usados 16 artigos no total, distribuídos em tabela e gráfico. Foi observado que há na literatura quanto ao surgimento de UTK em estágio de terminalidade de vida, podendo ser causada devido a falência de órgãos, a hipoperfusão cutânea e hipóxia tecidual, podendo indicar a morte do paciente em até 6 semanas; quanto as características da lesão, os autores concordam que esta comumente surge na região sacrococcígea, em formato de pera, borboleta ou ferradura, variando a coloração de acordo com o estágio em que se encontra, apresenta bordas irregulares e inicialmente surge como bolhas, abrasão ou áreas escurecidas da pele, podendo evoluir do estágio I ao IV em poucas horas, acometendo a epiderme, derme e tecido subcutâneo; quanto aos fatores de risco para surgimento são mencionados a anemia, alterações de nutrição, insuficiência respiratória, hipóxia, diabetes mellitus e doença renal; já no que se refere a abordagem terapêutica, não espera-se cura completa, o plano é voltado para cuidados paliativos. Foi possível observar também que é uma temática pouco abordada e explorada, uma vez que a escassez de estudos sobre a UTK. Assim, sugere-se que mais pesquisas sejam realizadas frente a abordagem deste tipo de lesão.

Abstract:
Kennedy Terminal Ulcer (UTK) is defined as a particular type of injury developed during the process of death. In view of the above, the objective the aim of this study is to evaluate the scientific production about THE UTK, punctuating its characteristics. The present study was a integrative literature review conducted through the bibliographic survey in medline database, PUBMED, Google Scholar. 16 articles were selected in total, distributed in table and chart. It was observed that there is in the literature regarding the appearance of UTK in the terminal stage of life, and may be caused due to organ failure, cutaneous hypoperfusion and tissue hypoxia, which may indicate the death of the patient within 6 weeks; as for the characteristics of the lesion, the authors agree that it commonly appears in the sacrococcygeal region, in the shape of pear, butterfly or horseshoe, varying the color according to the stage in which it is, presents irregular edges and initially appears as blisters, abrasion or darkened areas of the skin, and may evolve from stage I to IV in a few hours, affecting the epidermis , dermis and subcutaneous tissue; anemia, nutrition alterations, respiratory failure, hypoxia, diabetes mellitus and kidney disease are mentioned for risk factors for onset; with regard to the therapeutic approach, it is not expected complete cure, the plan is focused on palliative care. It was also possible to observe that it is a subject little addressed and explored, since the scarcity of studies on UTK. Thus, it is suggested that more research be conducted in the face of the approach of this type of injury.
