Abstract: Introduction: Fronto-naso-orbito-ethmoid fractures (FNOE) are complex injuries that involve the upper and middle center of the face and are difficult to diagnose and treat. For the diagnosis and planning of surgical treatment of these fractures, a detailed clinical examination is necessary, in addition to an image examination obtained through computed tomography (CT). The treatment aims to adequately restore the architecture of the frontal bone, the intercanthal distance, maintenance of the tear drainage system and restoration of facial aesthetics. Objective: To report a clinical case of FNOE fracture treatment, through fixation of bone fragments with titanium plates and screws. Case report: Patient with initials W. L. S., male, 22 years old, presented to the Oral and Maxillofacial Surgery and Traumatology service of the UFPI University Hospital with a history of falling from his height after a convulsive episode (SIC), 5 days after the accident. On clinical examination, it was possible to observe the presence of abrasions, asymmetrical deformity with bone depression in the frontal region, rhinoscoliosis on the right, pain on palpation in the face, preserved eye movements, and visual acuity. He showed no signs of fractures in his maxilla or mandible. The tomographic examination made it possible to confirm the diagnosis of fracture of the FNOE complex. The surgical approach was via coronal access, and subciliary on the E side, for reduction and fixation of the fractures using titanium plates and screws. During the 3-month post-surgery follow-up, adequate facial symmetry, patent airways, tear drainage system without signs of damage, and inconspicuous scars were found. Conclusion: Although the FNOE complex fracture is challenging for the oral and maxillofacial surgeon, the correct diagnosis associated with surgical planning and treatment allows for a more predictable aesthetic and functional result. The surgical technique and fixation materials used in the reported case proved satisfactory.
