Abstract: The article reviews the non-operative treatment of acute appendicitis with antibiotics, exploring therapeutic options beyond appendectomy. The integrative review analyzed 2,851 articles on non-operative treatment of appendicitis with antibiotics, selecting 35 for analysis after specific criteria for selection and analysis of the results found in the literature. Recent studies suggest that antibiotic therapy can be effective, with success rates ranging from 67% to 73.8% in one year, resulting in fewer days of disability compared to surgery. The non- operative approach can be a safe and effective alternative, especially in uncomplicated cases. Careful patient selection is essential for the success of this type of treatment. The Appendicitis Acuta (APPAC) II study and the CODA study showed promising results, with high success rates at one year. However, it is important to consider eligibility criteria and patient-reported outcomes when evaluating the effectiveness of treatment options. In summary, non-operative treatment of acute appendicitis with antibiotics is increasingly proving to be a viable option, with favorable results in terms of efficacy, safety, and patient satisfaction. Research continues to identify suitable patients for this type of treatment and to optimize long-term outcomes.
