Abstract: Introduction: Fractures in the zygomatic-maxillary complex cause relevant sequelae, with the area being considerably affected due to the projection of the middle third of the face. Objective: To report a case of fracture in the zygomatic-maxillary and frontal complex resulting from a motorcycle accident. Case report: Male patient, 27 years old, attended the Campina Grande Trauma Hospital after a motorcycle accident. Physical examination revealed injuries, periorbital ecchymosis (bilateral) and hyposphagma, maxillary and OPN crepitus under manipulation, identifying malocclusion, preserved visual acuity and ocular motricity, palpable step on the bilateral infraorbital margin. The diagnosis was zygomatic-maxillary complex fracture, frontal fracture, NOE type I fracture and OPN fracture. The intraoperative procedure consisted of orotracheal intubation with submental inversion, coronal access, complete maxillary vestibular access and access to the bilateral infraorbital margin. Maxillomandibular block was performed, reduction of the supraorbital rim and adaptation and fixation of a malleable mesh in the frontal region. There was fixation with mini plates and titanium screws of the left fronto-zygomatic pillar, the bilateral infra-orbital margin and the bilateral canine and zygomatic-maxillary pillar. Conclusion: It is necessary to use the correct techniques and effective planning, with image exams to prove the diagnosis. In this case, through post-operative examinations, the fixations performed proved to be effective.
