TERATOMA CÍSTICO MADURO: RELATO DE UMA APRESENTAÇÃO RADIOLÓGICA PATOGNOMÔNICA (“FLOATING BALLS”)
Objetivos: O trabalho teve como objetivo descrever um caso de teratoma cístico maduro de apresentação atípica e rara, porém de características patognomônicas. História Clínica: Paciente do sexo feminino, de 42 anos, sem comorbidades conhecidas, procurou atendimento médico com queixa de dor pélvica crônica. Foi realizada ressonância magnética (RM) da pelve que demonstrou lesão expansiva cística anexial bem definida, com conteúdo heterogêneo, apresentando múltiplas formações ovaladas regulares “flutuando” em meio ao componente cístico. Discussão e diagnóstico: O teratoma cístico maduro é a forma mais comum de neoplasia ovariana benigna, derivada de células germinativas. Caracteriza-se pela presença de tecidos bem diferenciados no seu interior e é facilmente reconhecido pelos métodos de imagem. O principal achado é a presença de gordura intratumoral, por isso também é chamado de cisto dermoide. Entretanto, apesar de raras, apresentações atípicas variadas podem ser observadas pela diversidade de tecidos envolvidos. Neste relato apresentamos um caso de teratoma com “esferas flutuantes”. Essas esferas podem ser formadas por debris sebáceos, cabelos, gordura e material proteico. O mecanismo de formação das mesmas permanece incerto, mas acredita-se que seja pela agregação de material sebáceo ao redor de um núcleo, que pela diferença de características físicas, adquiram o formato esférico e flutuem dentro do conteúdo cístico. Conclusões: Apesar da forma clássica do teratoma cístico maduro ser bastante reconhecível pelos métodos de imagem, as apresentações atípicas podem ser um desafio diagnóstico. Essa forma de apresentação até então não foi descrita em nenhum outro tipo de tumor, sendo patognomônico de teratoma cístico maduro, não devendo ser confundido com neoplasia maligna.

Abstract:
Objectives: This paper describes a case of mature cystic teratoma with an atypical and rare presentation but pathognomonic of this tumor. Clinical history: A female patient, 42 years old, without previous comorbidities, looked for medical attention complaining of chronic pelvic pain. The pelvic magnetic resonance imaging (MRI) showed an expansile cystic lesion well defined, with heterogeneous content and multiple “floating” regular round masses. Discussion and diagnosis: Mature cystic teratoma is the most common type of benign ovarian neoplasm, derived from germ cells. It is easily recognized by imaging methods because it contains well-differentiated tissues. The main feature is the presence of intratumoral fat, the reason why it is also called a dermoid cyst. Although rare, varied atypical presentations can be observed due to the fact that a wide variety of tissues may be present in the tumor. In this report, we present a case of teratoma with “floating balls”. These balls can be formed by sebaceous debris, hair, fat, and protein material. The mechanism of their formation remains uncertain, but it is believed that they are formed by the aggregation of sebaceous material around a nucleus that, by a difference in physical characteristics, acquire a spherical shape and float within the cystic content. Conclusions: Although the classic type of mature cystic teratoma is quite recognizable by imaging methods, atypical presentations can be a diagnostic challenge. This presentation has not been described in any other type of tumor yet, being pathognomonic of mature cystic teratoma and should not be confused with malignant neoplasm.
