Abstract: Treatment-resistant depression is characterized as a severe and difficult-to-manage psychiatric condition, requiring innovative therapeutic approaches that overcome the limitations of conventional antidepressants. This article aims to review the scientific evidence regarding the efficacy and safety of ketamine, esketamine, and neuromodulation techniques in the treatment of treatment-resistant depression. It is an integrative literature review based on studies published between 2015 and 2025 in the PubMed, Scielo, Embase, and Cochrane databases. Evidence indicates that ketamine and esketamine promote a rapid and significant antidepressant response, while neuromodulation shows sustainable results and a favorable safety profile. It is concluded that these innovative therapies represent a promising advancement, although the standardization of protocols and long-term follow-up remain important challenges.
