Abstract: Androgenetic alopecia (AGA) is a chronic hair loss condition that worsens when associated with telogen effluvium, making diagnosis and treatment more challenging. This study reports the case of a 37-year-old female patient with diffuse AGA and chronic telogen effluvium, influenced by hormonal, metabolic, and emotional factors. The patient presented with excessive scalp oiliness, perifollicular inflammation, and a history of prolonged use of hair extensions. Trichoscopic analysis revealed follicular miniaturization, diffuse thinning, and scalp dysbiosis. Treatment followed an integrative protocol with in-office sessions over four months, incorporating scalp massages, low-level laser therapy, blue LED light, ILIB (Intravascular Laser Irradiation of Blood), aromatherapy, and topical caffeine. Caffeine proved effective as a peripheral antiandrogenic agent, provided it was applied after proper follicular decongestion. After seven weeks, significant clinical improvement was observed, with reduced hair shedding and stimulated regrowth. The therapeutic success was attributed to the personalized care plan, consistent follow-up, and patient engagement. This case highlights the importance of integrative strategies in managing AGA, particularly in multifactorial and inflammatory conditions.
