Abstract: Sustainability has become a central axis for understanding contemporary public health challenges, particularly in light of the growing burden of noncommunicable diseases (NCDs) and the impacts of the environmental crisis on population living conditions. This chapter examines the interfaces between sustainability, NCDs, and Primary Health Care (PHC), adopting an expanded perspective of the social determinants of health and recognizing that the health–disease process is deeply shaped by social, economic, environmental, and territorial factors. Initially, sustainability is discussed as a social determinant of health, highlighting its relationship with environmental conditions, living standards, and health inequities, with particular attention to the disproportionate impacts on socially vulnerable populations. The chapter then addresses the epidemiological profile of NCDs in the context of the environmental crisis, emphasizing the roles of pollution, climate change, food systems, and urbanization in the development and worsening of chronic conditions. Sustainable diets are presented as a key strategy for NCD prevention and for reducing environmental impacts, underscoring the need for intersectoral actions. The chapter highlights PHC as a strategic pillar of sustainable health systems, emphasizing care practices grounded in health promotion, longitudinal follow-up, rational use of resources, and intersectoral coordination. Finally, challenges and future perspectives are discussed, reinforcing that strengthening PHC, guided by the principles of sustainability and equity, is essential for building more resilient, just, and responsive health systems capable of addressing present and future health demands.
