SUPORTE DE VIDA AO POLITRAUMATIZADO
Objetivo: Evidenciar a importância de um atendimento inicial sistematizado na redução da morbimortalidade em pacientes politraumatizados, que é considerado a terceira maior causa de morte no mundo e a primeira causa de morte nos adultos jovens no Brasil. Métodos: Consiste em uma revisão de literatura sobre a melhor abordagem inicial de um paciente vítima de trauma, bem como definir a conduta imediata. Foram selecionados artigos nas bases de dados PubMed, Medline, Lilacs e SciELO publicados entre 2015 e 2021. Houve uma análise criteriosa dos artigos selecionados para essa revisão através dos descritores definidos, que são: “ Serviço médico de emergência”, “Cuidados de suporte avançado de vida no trauma”, “Traumatismo múltiplos” e “Atendimento pré-hospitalar”. Foram selecionados artigos em inglês, espanhol e português. Resultados: Uma das definições mais recentes de politrauma é “paciente vítima de trauma com duas lesões ou mais, sendo uma delas potencialmente ameaçadora à vida.” Devido a isso, o atendimento inicial desses pacientes devem ser sistematizado de acordo com o mnemônico ABCDE (Airway, Breathing, Circulation, Disability, Exposure) que define as prioridades no atendimento e possibilita o reconhecimento e tratamento precoce das lesões que possam levar a óbito. Considerações Finais: O politraumatizado é complexo e o conhecimento sobre a estrutura sistematizada do ABCDE, bem como o reconhecimento e tratamento rápido de lesões ameaçadoras à vida têm como objetivo diminuir drasticamente a morbimortalidade desses pacientes.

Abstract:
Objective: To highlight the importance of systematic initial care in reducing morbidity and mortality in polytrauma patients, which is considered the third leading cause of death in the world and the leading cause of death in young adults in Brazil. Methods: It consists of a literature review focusing on the best initial approach to a trauma patient, as well as defining its prompt management care. Articles were selected from PubMed, Medline, Lilacs and SciELO databases, all of each published between 2015 and 2021. There was a careful analysis of the selected articles for this review through the defined descriptors, which are: “Emergency medical service”, “Advanced trauma life support”, “Multiple trauma” and “Pre-hospital care”. Articles in English, Spanish and Portuguese were thereby selected. Results: One of the more recent definitions of polytrauma is consisted by “two or more associated injuries, one of which that is potentially life threatening.” Thereby, the initial care for these following patients should be systematized according to the ABCDE mnemonic (Airway, Breathing, Circulation, Disability, Exposure), which of whom sets priorities in care and enables the early recognition and treatment of injuries that could lead to death. Final considerations: Polytrauma patients are complex and the knowledge about the systematized structure of the ABCDE, as well as the prompt perception and given treatment of life-threatening injuries aim to drastically reduce the morbidity and mortality of these patients.
