Abstract: Implant placement in the posterior region of the maxilla is a common clinical practice, but this area requires special care, as accidents can more easily occur due to its peculiarities, such as perforation of the maxillary sinus membrane and implant displacement into the maxillary sinus, both situations potentially leading to infectious consequences such as acute or chronic sinusitis. The authors present the case of patient L.C.B., 55 years old, with a history of maxillary sinus lift surgery for implant placement and fabrication of a protocol-type prosthesis. Eight months after surgery and prosthesis installation, the patient developed complaints of foul odor in the oral cavity, drainage of purulent secretion, and associated pain. Upon physical and imaging examination, the presence of a fistula through one of the implants was verified as the entry point for the development of chronic sinusitis. Surgical treatment was proposed through maxillary sinus surgery under local anesthesia and intravenous sedation. The authors conclude that the posterior region of the maxilla is a delicate area for implant placement due to factors such as low density and limited bone availability due to alveolar crest resorption and maxillary sinus pneumatization, underscoring the importance of careful planning to prevent complications in the perioperative and postoperative periods.
