Abstract: Introduction: Sinusitis is the inflammation of the paranasal sinuses and one of the most common respiratory conditions in childhood. Its etiology may be viral or bacterial, usually presenting a self-limiting course; however, it can progress with complications that may lead to severe clinical outcomes. These complications can range from local manifestations, due to contiguous spread of infection, to systemic involvement, which may result in unfavorable outcomes for the patient. Sinusitis is an epidemiologically relevant disease, with straightforward diagnosis and treatment. Recognizing its signs and symptoms is essential for healthcare professionals, since the diagnosis is primarily clinical and does not require complementary tests. Early diagnosis prevents adverse outcomes, reducing the risk of sequelae and death. Objective: To report a case of severe complications resulting from bacterial sinusitis diagnosed and treated late. Methods: Case report based on the patient’s medical records, supported by a retrospective literature review using global databases including SciELO, PubMed, and Google Scholar. CAAE: 87538825.3.0000.5514. Case report: We describe the case of a 7-year-old male patient, E.H.M.S.C, diagnosed with preseptal cellulitis and subdural empyema as complications of sinusitis diagnosed late at the referring service. The patient remained hospitalized in our service from January 30, 2024, to February 28, 2024. Conclusion: Sinusitis in children is a common condition with significant clinical relevance that requires careful evaluation for early diagnosis and treatment to prevent severe complications and sequelae.
