Abstract: Silybum marianum L. (Asteraceae), popularly known as "milk thistle," is a medicinal plant of Mediterranean origin, widely used in the treatment of liver diseases. The aim of this review was to identify the therapeutic effects of S. marianum extract (silymarin) on liver disorders. The methodology consisted of an integrative review in SciELO, LILACS, PubMed, and BVS databases, using the descriptors "Silymarin," "Liver Diseases," and "Non-Alcoholic Fatty Liver Disease (NAFLD)". The searches were conducted from 2019 to 2024. Fifty-two articles were found, of which 13 met the inclusion criteria. The results indicate that silymarin has beneficial effects in reducing liver enzymes, SGOT (Serum Glutamic-Oxaloacetic Transaminase), SGPT (Serum Glutamic-Pyruvic Transaminase), and GGT (Gamma-Glutamyl Transferase). Silymarin also improves liver function and has a favorable safety profile, particularly in cases of NAFLD, viral hepatitis, and alcoholic liver disease, demonstrating hepatoprotective, antioxidant, and anti-inflammatory functions. In conclusion, silymarin stands out as a promising therapeutic alternative.
