SEGURANÇA DO PACIENTE E COMUNICAÇÃO NA PASSAGEM DE PLANTÃO DA ENFERMAGEM: EXPERIENCIA NO USO DA METODOLOGIA SBAR
Passagens de plantão adequadas são objetivas, precisas e permitem que os profissionais de enfermagem conheçam a evolução clínica dos pacientes e tenham uma análise geral da unidade de assistência. Em uma Unidade de Terapia Intensiva (UTI), foi observado que durante as passagens de plantão, existiam inconformidades no repasse de informações relevantes. O objetivo deste estudo foi relatar a experiência da implantação de metodologia estruturada de comunicação durante as passagens de plantão na UTI Neonatal de um hospital de ensino no Mato Grosso do Sul. Para tal, foi realizada uma revisão bibliográfica acerca da comunicação efetiva e verificou-se a descrição da metodologia Situação / Breve histórico / Avaliação / Recomendação (SBAR). A metodologia SBAR foi selecionada com a aprovação dos profissionais envolvidos, por ser recomendada pela The Joint Commission e pela Rede Brasileira de Enfermagem e Segurança do Paciente. Foi elaborado treinamento da equipe, construção da versão digital e estruturada da metodologia SBAR. Durante a experiência de implantação e por sugestão da equipe, foram realizadas alterações na metodologia SBAR, assim foi acrescentado o item “Identificação”. Ocorreu a inversão dos itens “Situação” e “Breve histórico” e a unificação dos itens “Avaliação” e “Recomendação”. Um arquivo digital foi disponibilizado para impressão diária no período noturno, com os itens atualizados da “Identificação”, “Breve histórico” e “Situação” preenchidos. Os demais elementos de “Avaliação” e “Recomendação”, foram inseridos manualmente a cada passagem de plantão. A experiência demonstrou que ocorreu a flexibilização do método e a adaptação às necessidades específicas da unidade. O uso da metodologia SBAR mitigou os riscos de falhas, por ser um método vinculado ao roteiro escrito, situação que permitiu um modelo mental compartilhado em torno de todo o quadro clínico dos pacientes.

Abstract:
Adequate changes of shifts are objective, accurate, precise, and allow professionals of the nursing department to know and fully understand the clinical evolution of patients as well as have a general analysis of the assistance unity. In an Intensive Care Unit (ICU), it has been observed that during the changes of shifts, there were inconsistences when it comes to sharing relevant information. The aim of this study was to report the experience of the implementation of the structured methodology of communication in between the changes of shifts in the Neonatal ICU of a teaching hospital in the state of Mato Grosso do Sul. There for, a bibliography review was made in regards of the effective communication, when it was noticed the description of the methodology Situation, Background, Assessment, Recommendation (SBAR). The SBAR methodology was selected with the approval of the professionals involved for being recommended by The Joint Commission and the Brazilian Network of Nursing and Patient Safety. A training for the crews was elaborated and a digital structured version of the SBAR methodology was made. During the implementation of the experience and as a suggestion of the team, changes were made in the SBAR methodology, when an addition was done and the item “Identification” was added. An inversion of the items “Situation” and “Background” was also done, and finally, a merge of the items “Assessment” and “Recommendation”. A digital file was made available for daily printing during night shifts with the updated items “Identification”, “Background” and “Situation” already filled out. The other elements “Assessment” and Recommendation”, were filled manually in each shift. The experience showed that there was a flexibilization on the method and some adjustments considering the specifics needs in every unity. Being a method linked to a written script, the use of the SBAR methodolody minimized the risks of failure, which allowed a shared mental model built around the patient clinical state.
