Abstract: Infertility is a global health issue affecting 8–15% of couples of reproductive age, with multifactorial causes including endocrine disorders, metabolic, genetic, and environmental factors. This chapter provides a comprehensive review of advances in reproductive endocrinology, diagnosis, and therapeutic strategies for infertility management. Topics include the physiology of the hypothalamic–pituitary–gonadal axis, the role of biomarkers such as anti-Müllerian hormone (AMH) and inhibin B, and the integration of technologies like artificial intelligence, genetic sequencing, and assisted reproductive technologies (ART). Innovations such as in vitro oocyte maturation, fertility preservation, regenerative therapies with stem cells and platelet-rich plasma, and personalized approaches involving endocrinologists, gynecologists, andrologists, and embryologists are highlighted. Future perspectives address gene editing with CRISPR-Cas9, 3D bioprinting of reproductive tissues, and advances in cryopreservation. By emphasizing patient-centered, multidisciplinary, evidence-based reproductive medicine, the chapter outlines strategies to maximize success rates, reduce risks, and alleviate the emotional and financial burden of infertility.
