Abstract: 1. INTRODUCTION: Breast cancer is the leading cause of cancer-related death among women in Brazil, and mutations in the TP53 gene are associated with tumor predisposition, such as Li-Fraumeni syndrome. 2. CASE REPORT: A 27-year-old woman with a palpable breast nodule, associated with pain, local hyperemia, and rapid growth. Breast biopsy revealed breast sarcoma. Family history included brain cancer and leukemia. 3. RESULTS: Population-based risk assessment tools estimated a 1.15% risk for cancer. However, when the TP53 mutation was considered, the cumulative lifetime risk of breast cancer exceeded 60% by the age of 85. 4. DISCUSSION: Li-Fraumeni syndrome (LFS) is associated with a characteristic spectrum of neoplasms. Surveillance strategies and prophylactic procedures should be considered essential in patient management. 5. CONCLUSION: This report highlights the role of oncogenetics in clinical practice, contributing to early diagnosis, improved prognosis, and quality of life for individuals with LFS.
