Abstract: Introduction: Diabetes mellitus (DM) is a chronic metabolic disease characterized by persistent hyperglycemia resulting from insufficient insulin production or the body's inability to use insulin effectively. Among its complications, diabetic neuropathy (DN) stands out and is associated with an increased risk of falls, especially in older adults. Aim: To investigate the association between fall risk and sociodemographic factors in patients with diabetic neuropathy who are inactive in the healthcare system of the municipality of Guarapuava, Paraná, Brazil. Methods: This is an epidemiological, descriptive, cross-sectional study with a quantitative approach, conducted with individuals with DM registered in Primary Health Care Units in Guarapuava, Paraná. The variables analyzed included diabetic neuropathy (Neuropathy Disability Score – NDS), sociodemographic factors (sex, age, education level, race/ethnicity, marital status, and income), and fall risk assessed using the Timed Up and Go Test (TUG). Association analyses were performed considering a significant level of 5%. Results: The sample consisted of 296 participants, predominantly female, with a mean age of 66.9 years, low educational level (one to four years of schooling), self-reported white race/ethnicity, living with a partner, and income above one minimum wage. A statistically significant association was observed between neuropathic impairment and the variables sex (p<0.01), education level (p<0.01), income (p<0.01), and fall risk (p<0.001). Conclusion: Socioeconomic conditions and fall risk are associated with the presence of diabetic neuropathy, highlighting the importance of preventive strategies and clinical follow-up targeting these factors in Primary Health Care.
