Abstract: Transsurgical restorations are alternative procedures that seek to obtain an ideal restorative access for a post-surgical with restored aesthetics and preservation of periodontal structures in teeth with unfavorable situations. This work aims to report a clinical case of transsurgical restoration of a subgingivally fractured tooth. Patient with dental unit 12 presenting fracture from the incisal edge to the cervical third, with subgingival fracture margin. After performing clinical and radiographic examinations, it was observed the need for a clinical crown increase prior to the restoration of this unit. At first, a clinical crown increase was performed, obeying the height of 3 mm of the biological space. After that, the preparation was made in the tooth so that a fiberglass pin was produced, and thus, the provisional restoration with composite resin on the pin immediately after the surgery, with the use of an adequate and efficient absolute insulation to obtain a satisfactory result. It is concluded that, for the rehabilitation treatment to be satisfactory, it is necessary to be attentive to the patient's periodontal conditions, respecting the supracrestal insertion tissues and the segregation of the oral environment with the dental unit to be rehabilitated, through effective absolute isolation, which results in better results, ensuring longevity for restorative treatments.
