Abstract: The aesthetic demands of patients and professionals are increasing nowadays, which has led Dentistry to seek innovative alternatives for restorative techniques and improvement of restorative materials. Indirect restorations with composite resin in posterior teeth are performed using an extraoral method, facilitating the achievement of efficient proximal contact, facilitating the performance of occlusal anatomy and the finishing and polishing stages. This work means a clinical case report about an onlay restoration built on a silicon cast. It means an alternative for repairing teeth with considerable loss of tooth structure, aiming to achieve a well-adjusted restoration, with a satisfactory and long-lasting outcome. Although it is a relatively new technique, the indirect restoration built on silicon cast has proven to be effective, with good marginal adaptation, more precise anatomical characterization and lower cost when compared to porcelain onlay, being an alternative to conventional direct composite restoration.
